Diffuse hypertrichosis during treatment with 5% topical minoxidil

Abstract: Five women affected by androgenetic alopecia developed severe hypertrichosis of the face and limbs after 2-3 months of treatment with 5% topical minoxidil. Minoxidil was discontinued and in all patients the hypertrichosis disappeared from the face and arms after 1-3 months, and from legs after 4-5 months. Systemic absorption of minoxidil, and a high sensitivity to minoxidil of the follicular apparatus in these areas, is hypothesized.

“…Side‐effects of minoxidil treatment include allergic contact dermatitis, which precludes further use of the drug and reversible hypertrichosis. This is usually seen with the 5% concentration 30 and can involve the face, indicating local contamination, or distal regions such as the arms and the buttocks ( Fig. 10), suggesting drug absorption.…”

“…Occasionally it causes scalp irritation which may be severe enough to cause a temporary increase in hair shedding and patients should be warned about this. Hypertrichosis on the face and on more remote sites has been reported, particularly when higher concentrations of minoxidil are used 37 . This resolves if treatment is stopped.…”

“…The topical form reduces alopecia by increasing circulation and follicular DNA synthesis. Topical minoxidil results in localized hair growth, with only 0.5% to 5% of patients reporting generalized hypertrichosis . It is postulated that hypertrichosis results from systemic absorption combined with follicle hypersensitivity.…”
