Differentiating Atrioventricular Nodal Re-Entrant Tachycardia From Junctional Tachycardia

Abstract: T ypical atrioventricular nodal re-entrant tachycardia (AVNRT) is the most common supraventricular tachycardia; however, junctional tachycardia (JT) is rare and occurs mostly in children or during infusion of isoproterenol. 1,2 Distinguishing between these arrhythmias can often be challenging during electrophysiology testing. They share many common characteristics, including similar intracardiac activation patterns, similar sites of earliest activation, and typical occurrence of a His bundle deflection before … Show more

“…Furthermore, JET may occur during isoproterenol infusion after slow pathway ablation and failure to diagnose of this arrhythmia may lead to further unnecessary ablation with an increased risk of AV block. 1,2,21 Failure to entrain tachycardia favors a nonreentrant mechanism.…”

“…During EP study, both arrhythmias have similar intracardiac electrogram activation patterns, similar HV intervals, and similar sites of earliest activation during catheter mapping (Figure 1B), and they may also be associated with VA block. Furthermore, JET may occur during isoproterenol infusion after slow pathway ablation and failure to diagnose of this arrhythmia may lead to further unnecessary ablation with an increased risk of AV block 1,2,21 …”

“…An atrial extrastimulus that advances the His potential immediately after it without terminating the tachycardia indicates that the retrograde fast pathway is not essential for the circuit and suggests the diagnosis of junctional tachycardia. This criterion, however, requires that no dual (or multiple) response of the AV node is present and this is not always the case . Atrial overdrive pacing has also been proposed for differentiation of JT from AVNRT with an AHHA response versus an AHA response, respectively, provided there is no dual AVN physiology in the presence of JT .…”
