Purpose Despite management, some patients continue to have bothersome lower urinary tract symptoms (LUTS). We examined characteristics associated with LUTS bother in a prospective cohort. Materials and Methods Data were obtained from care-seeking patients with LUTS at six US tertiary-care centers in a one-year prospective, observational cohort study. Participants answered the American Urological Association Symptom Index global urinary bother question at study entry and 12 months later. Multiple linear and logistic regressions were used to identify factors associated with 12-month urinary bother. Results Of 756 participants, 121 (16%) had worsened LUTS bother over the study period. Baseline factors associated with more severe bother at 12 months among men included non-white race, hypertension, worse urinary frequency and incontinence, and higher levels of stress (all p<0.05). Among women, more severe bother at baseline, urinary urgency and frequency, and worse physical function were associated with more severe bother at 12 months. Adjusted for other variables, worsened LUTS were more likely among men who were non-white (OR [95% CI]=1.79 [0.95–3.39]) or diabetic (OR=1.68 [0.86–3.26]) and among women with diabetes (OR=1.78 [0.86–3.65]), prior LUTS treatment (OR= 2.59 [1.24–5.40]), or higher levels of depression (OR=1.30 [1.11–1.53]). Conclusion Urinary symptom severity at baseline, race, depression, and psychological stress were associated with LUTS bother in a prospective cohort of men and women treated at tertiary-care facilities. These findings may inform the clinical care of patients with bothersome LUTS and direct providers to better prognosticate challenging LUTS cases.

Resumen Objetivo: evaluar la evidencia científica sobre la efectividad de las terapias complementarias para el control de los síntomas del tracto urinario inferior en la población masculina adulta y adulta mayor. Método: revisión sistemática desarrollada según la checklist PRISMA. La búsqueda se realizó en las bases de datos CINAHL, Embase, LILACS, PEDro, PubMed, Web of Science y Google Scholar. Resultados: se identificaron 585 registros y se seleccionaron 12 ensayos clínicos que cumplían con los criterios de inclusión. Los resultados que los estudios consideraron para analizar la efectividad de las terapias complementarias fueron cuestionarios validados que evaluaban la gravedad de los síntomas del tracto urinario inferior (sensación de vaciado incompleto de la vejiga, micción frecuente, flujo intermitente, flujo débil, dolor o dificultad para orinar, nicturia y urgencia) y parámetros urodinámicos. Los estudios analizaron las terapias complementarias fitoterapia (n=8) y electroacupuntura (n=4). Seis estudios relacionados con la fitoterapia demostraron significación estadística. La electroacupuntura demostró una mejoría significativa de los síntomas en dos estudios. Conclusión: la fitoterapia fue efectiva para controlar los síntomas de frecuencia, urgencia, nicturia, vaciado incompleto, intermitencia, flujo débil y esfuerzo para iniciar la micción. Para confirmar la efectividad de la electroacupuntura, aún es necesario que se realicen investigaciones con metodologías bien diseñadas para resolver las diferencias entre los estudios de esta revisión.

After the approval and introduction of mirabegron, tadalafil, and botulinum toxin A for treatment of lower urinary tract symptoms/overactive bladder, focus of interest has been on their place in therapy versus the previous gold standard, antimuscarinics. However, since these agents also have limitations there has been increasing interest in what is coming next - what is in the pipeline? Despite progress in our knowledge of different factors involved in both peripheral and central modulation of lower urinary tract dysfunction, there are few innovations in the pipe-line. Most developments concern modifications of existing principles (antimuscarinics, β3-receptor agonists, botulinum toxin A). However, there are several new and old targets/drugs of potential interest for further development, such as the purinergic and cannabinoid systems and the different members of the transient receptor potential channel family. However, even if there seems to be good rationale for further development of these principles, further exploration of their involvement in lower urinary tract function/dysfunction is necessary.
