Differential diagnosis of periampullary carcinomas: comparison of CT with negative-contrast CT cholangiopancreatography versus MRI with MR cholangiopancreatography

Abstract: The CT set provides a comparable performance to that of the MRI set in differentiating periampullary carcinomas, and it may be an alternative to 2D-MRI with MRCP in assessing malignant biliary obstruction in patients who are not suitable for MR examinations.

“…But its value is still unsatisfying in the diagnosis in periampullary lesions. Previous studies have reported that the specificity was 75.68-100%, sensitivity was 60-100%, and accuracy was 91.17-92.92% in the diagnosis of periampullary space-occupying lesions [11][12][13][14]. Then, MRCP examination requires a magnetic field, which is contradicted for patients with metal implanted material, such as cardiac pacemaker, metal stent, etc.…”

“…In addition, MRI requires a magnetic field that is contradicted for patients with cardiac pacemakers, metal clips, or metal stents [8]. Previously, the specificity and accuracy of MRI for PSOLs were found to be 78.3 and 91.5%, respectively [6, 11].…”
