Differential Deposition of C4d and MBL in Glomeruli of Patients with ANCA-Negative Pauci-Immune Crescentic Glomerulonephritis

Abstract: Complement activation was involved in renal damage of human ANCA-negative pauci-immune CrGN but with heterogeneous activation pathways. Positive staining of C4d and MBL might be associated with poor renal outcome.

“…Data about C4d renal deposition in AAV are scarce. A previous small study on seven ANCA-positive patients (7) documented C4d negativity in NCGN, but later C4d was found to coexist with C3d in renal biopsy specimens from patients with ANCA-negative pauci-immune NCGN (16). In this study, the activation of the complement lectin pathway was hypothesized as superimposed on the activation of the cAP via the production of C3b.…”

“…The involvement of MBL in glomeruli was reflected by a poor renal prognosis, e.g. more severe renal disease in cases of IgA nephropathy (Roos et al [19]) and in patients with antinuclear antibody-negative Pauci-immune crescentic glomerulonephritis (Xing et al [20]). In dialysis patients (Ibernon et al [21]), low pretransplant levels of MBL were associated with chronic inflammation, new-onset diabetes mellitus after transplantation, and infection.…”

“…C4d is widely used as marker of antibody-mediated rejection, 14,15 and studies have also shown that it is positive in immune complex-or Ig-mediated GN, such as lupus nephritis, membranous nephropathy, antiglomerular basement membrane GN, and antineutrophil cytoplasmic antibody crescentic GN. [16][17][18][19][20][21][22] Recently, it was shown that C4d staining in IgA nephropathy might serve as a poor prognostic factor. 11 However, studies have not been done regarding its potential role in the diagnosis of C3 glomerulopathy.…”
