Different solubilizing ability of cyclodextrin derivatives for cholesterol in Niemann–Pick disease type C treatment

Yusei Yamada,
Madoka Fukaura‐Nishizawa,
Asami Nishiyama
et al.

Abstract: BackgroundNiemann–Pick disease type C (NPC) is a fatal neurodegenerative disorder caused by abnormal intracellular cholesterol trafficking. Cyclodextrins (CDs), the most promising therapeutic candidates for NPC, but with concerns about ototoxicity, are cyclic oligosaccharides with dual functions of unesterified cholesterol (UC) shuttle and sink that catalytically enhance the bidirectional flux and net efflux of UC, respectively, between the cell membrane and the extracellular acceptors. However, the properties… Show more

“…Hydroxypropyl-β-cyclodextrin (HPβCD) is a cholesterol chelator that has been shown to be effective in treating lipid metabolism disorders, such as Niemann-Pick disease ( Yamada et al, 2021 , 2022 , 2023 ). However, it has been found to have a severe damaging effect on patients’ hearing.…”

“…The formation and accumulation of CD-cholesterol or CD-CCs complexes may lead to further decline in solubility and even precipitate in aqueous systems, which may be a significant factor in causing acute renal toxicity by intravenous administration [ 28 , 29 ]. Consequently, β-CD may also easily and efficiently extract plasma membrane-bound cholesterol, which can greatly induce hemolysis and increase ototoxicity [ 30 , 31 ]. Therefore, β-CD has been chemically modified to improve solubility and biocompatibility for clinical applications.…”
