Background: We herein report a rare case of a sclerosing stromal tumor (SST) in an adolescent. In this case, the mass displayed a shrinking trend, combined with its ultrasonic manifestations and pathological characteristics and may provide some references for the selection and timing of treatment, to avoid excessive harm to patients.Case Description: A healthy 17-year-old adolescent female presented to the outpatient department, complaining of abnormal uterine bleeding, but no abdominal pain, bloating, chills, or fever. The patient had no history of malignant tumors, and no relevant family or genetic history. An ultrasound showed an inhomogeneous hypoechoic area (106 mm ×53 mm ×68 mm) in the right ovarian, a clear boundary, an anechoic area inside and blood flow was observed in the mass. At a follow-up regular re-examination, the mass displayed a shrinking trend from 95 mm ×50 mm ×88 mm, 61 mm ×28 mm ×42 mm, 43 mm ×28 mm ×40 mm, 43 mm ×28 mm ×40 mm, to 42 mm ×23 mm ×28 mm. The patient underwent laparoscopic surgery a week later. Based on the immunohistochemistry and morphology results, the posterior ovarian mass was diagnosed as a SST. At one month after operation, there was no obvious abnormality on ultrasound. Conclusions:The incidence of SST is relatively low. However, due to the low specificity of clinical manifestations, imaging examination and serum tumor markers, the diagnosis of SST mainly relies on pathological examination. Therefore, in clinical practice, the possibility of misdiagnosis is greater, and attention should be paid to the differentiation of ovarian malignant tumors. Surgical resection is recommended, and the effect is good. Surgical methods should be selected individually according to the size of the tumor and the age of the patient.

Sklerozan stromal tümörler (STT), genellikle yaşamın 2-3. dekadında genç kadınlarda görülen nadir benign over tümörleridir. Over stromal tümörlerinin %2-6’sını oluştururlar. Çalışmamızda, İzmir Tepecik Eğitim ve Araştırma hastanesi patoloji bölümünde 2011-2020 yılları arasında tanı alan 7 sklerozan stromal tümör olgusunu, klinik ve histopatolojik bulgularını tekrar gözden geçirerek literatür eşliğinde sunduk . Hastalarımızın yaş ortalaması 34.4 olup 2 hasta postmenopozal dönemdeydi. Olguların 4’ünün klinik takibi mevcut olup 1 hastada operasyondan 7 yıl sonra nüks görüldü. Makroskopik olarak tümörlerin çoğu tamamıyla solid görünümdeydi. Mikroskopik olarak tüm olgularda ödemli ve /veya kollojenden zengin hiposellüler alanlarla ayrılan hipersellüler alanlardan oluşan psödolobüler düzenlenim mevcuttu. Tümör, iğsi şekilli hücreler ve bol berrak – eozinofilik şekilli yuvarlak hücrelerden oluşuyordu. Çok nadir görülmeleri nedeniyle SST de preoperatif tanı mümkün değildir. Özellikle frozen çalışılan olgularda tipik histopatolojik bulgularla doğru tanı konulması, genç kadın hastalarda fertilite koruyucu cerrahinin uygulanabilmesi açısından önem taşır.

Sklerozan stromal tümör (SST) over seks kord stromal tümörleri arasında yer alan oldukça nadir görülen benign bir stromal tümördür. Bu olgu sunumunda sağ overde SST saptanan hastanın, radyolojik bulguları ve histopatolojik özellikleri literatür eşliğinde kısaca tartışılmıştır. 23 yaşında kadın hasta, pelvik ağrı şikayeti ile başvurduğu dış merkezde yapılan abdominal ultrasonografisinde sağ adnekste kistik ve solid komponenti olan kitle lezyon saptanması üzerine ileri tanı ve tedavi için hastanemiz kadın doğum polikliniğine yönlendirildi. Cinsiyet değiştirme istemi de olan hastaya yapılan laboratuvar incelemelerinde herhangi bir hormonal anormallik saptanmadı. Kitlenin frozen incelemesi sonrasında total abdominal histerektomi ve bilateral salpingoooferektomi (TAH+BSO) yapılması kararlaştırıldı. Frozen kesitlerin incelenmesi sonucu "seks kord stromal tümör" olarak değerlendirilen tümörün mikroskopik incelemesinde, kollajenöz ve ödemli hiposellüler alanlar ile hipersellüler alanların oluşturduğu psödolobüler patern, belirgin vasküler proliferasyon izlendi. Morfolojik ve immunhistokimyasal bulgular ışığında sağ overde izlenen tümör "SST" olarak tanı almıştır. Sklerozan stromal tümör olgularının büyük bir kısmı vasküler açıdan zengin solid kitle olduğundan radyolojik olarak öncelikle malign tümör olasılığı akla gelmektedir. SST'nin benign biyolojik davranışa sahip olması ve konservatif cerrahinin gerekli olduğu genç hastalarda görülmesi nedeniyle intraoperatif patolojik tanısı oldukça önemlidir.
