Differences in Middle Ear Ventilation Disorders Between Pars Flaccida and Pars Tensa Cholesteatoma in Sonotubometry and Patterns of Tympanic and Mastoid Pneumatization

Abstract: Significantly different tubal function and mastoid pneumatization patterns suggest differences in ventilation disorders and cause between ears with pars flaccida and pars tensa cholesteatoma.

“…Mastoid maturation was measured using temporal bone computed tomography (CT) as described by Iino et al (8,9). The pneumatized area, regardless of the presence or absence of aeration, was measured at the level of the lateral semicircular canal on axial CT using Ziostation version 1.1 software (Ziosoft Inc., Tokyo, Japan).…”

“…2,4 Thus, the degree of temporal bone pneumatisation is suggested to be due to the degree of Eustachian tube ventilation. 8 Pneumatisation is further contributed to by the extension of pneumatised mastoid antrum and mastoid air cells to the apex through multiple tracts around the otic capsule. [9][10][11] These tracts have been described as supralabyrinthine, infralabyrinthine, posteromedial, anterior and along the arcuate artery.…”

“…According to the literature, the development of otitis media and the formation of cholesteatomas may be related to the degree of pneumatization of the temporal bone. However, it is not known whether poor pneumatization is the cause or the result of otitis media [ 1 2 ].…”
