Die Zuggurtung der Symphysenruptur mit geflochtenen Polydioxanon-Bändern (PDS®)

Abstract

In seven patients (mean age 22 years) with injuries of the pelvic ring, the rupture of the symphysis was treated by polydioxanone ligaments. Complete rupture of the ileosacral joint was treated twice by additional external fixation, twice a compression screw osteosynthesis was applied. Incomplete iliosacral rupture in another three cases was not operated. Patients were treated functionally without signs of loosening or instability. After a control examination after 5 months all patients were free of symptoms in the area of the symphysis. Infection of the operation area did not accur. In our opinion the advantages of this method are simple operative technique, reduced risk of infection and no need for removal of metallic implants. Our excellent initial experience with synthetic PDS ligaments in operative treatment of symphysis ruptur seem to justify its application for other injuries like iliosacral or acromioclavicular rupture.

Suture Fixation of a Pediatric Pelvic Ring Injury

“…Although suture fixation has been widely used by pediatric urologists in the setting of bladder exstrophy 12,13 , there is little mention within the orthopaedic literature regarding its use for traumatic pelvic ring injuries. Two case series 14,15 and a biomechanical analysis of polydioxanone suture (PDS) fixation of the anterior pelvis 16 were published out of Germany in the late 1980s and early 1990s. Contrasted with PDS, FiberWire and FiberTape are nonabsorbable sutures, with a long track record of use in various orthopaedic procedures.…”

“…The patient did not sustain injury to his head, spine, or viscera, although the CT pelvis was notable for a pelvic hematoma. Laboratory evaluation revealed minimal abnormalities, with only leukocytosis (23.0 K/mL, ref [6][7][8][9][10][11][12][13][14][15][16][17], mildly elevated creatine kinase (588 IU/L, ref 32-250), and aspartate aminotransferase (69 IU/L, ref 7-40). The patient was placed in a long leg splint and sheet pelvic binder by the orthopaedic on-call resident.…”
