Background. The prevalence of synchronous or metastatic tumors in patients with head and neck squamous cell carcinoma (HNSCC) ranges from 6% to 20% and has implications for prognosis and management of the primary disease. There is no consensus about the role of chest CT prior to definitive treatment patients with HNSCC.Methods. A systematic review of all chest CT studies in relation to HNSCC was performed, together with a review of our local database.Results. Twenty-four studies were identified in addition to our local data. Prevalence of positive chest CT was 7.93%. Patients were significantly more likely to have a positive chest CT with N2 or N3 neck disease (p ¼ .0062) and stage III or IV disease (p ¼ .0001), and significantly less likely with tumors of the oral cavity (p ¼ .0007).Conclusion. We advocate chest CT as part of the initial investigations for patients with HNSCC.

The aims of follow-up of head and neck cancer patients are the detection of new tumour manifestations, management of impairments after tumour therapy, psychological care and the evaluation of therapeutic efficacy. The extent, success and cost-benefit ratio of follow-up are currently under discussion. We recommend interdisciplinary cooperation between the relevant specialists, such as oncology and radiotherapy, together with the otorhinolaryngologist for reasons of cost-efficacy and improvement of long-term results. We present a follow-up schedule for patients with head and neck squamous cell carcinomas, which are the by far most common manifestation. We recommend a standardized protocol, which should be individualized depending on tumour site, size, treatment and therapeutic options in the case of tumour recurrence. The most common salivary gland malignancies are also discussed. The objective is to increase the efficacy of follow-up in patients with head and neck cancer.

In 20 (13%) patients chest abnormalities were observed on CT scan of the chest and in 6 (4%) patients malignancy was pathologically confirmed. Both pulmonary second primary tumors and pulmonary metastases were independent of stage of oral malignancy. We found that additional diagnostic procedures did not significantly lengthen the time interval between first consult and start of treatment. The cost of the screening for pulmonary malignancies in the group was € 8.214 per observed pulmonary malignancy.We advocate that CT imaging of the chest should be routinely performed in the diagnostic work up of all patients with a newly discovered SCC of the oral cavity, irrespective of the tumor stage of the oral malignancy.
