The number of displaced persons today is the highest since World War II. Flight has become increasingly dangerous and refugees are now in poorer health conditions. Severe traumatization happens in their home-countries but atrocities under flight is increasing, including torture, trafficking and death. It has become increasingly difficult to reach Western countries and those who come now meet difficult conditions with poor living conditions and growing xenophobia. There are unmet treatment needs in the refugee population and lack of early preventive measures. Many severely traumatized will also need qualified psychotherapy. This article presents a psychoanalytic view on traumatization and describes specific features of psychoanalytic therapy with severely traumatized refugees. A case example is presented to illustrate how the analyst inevitably becomes involved in traumatization related mental scenarios and how this involvement may provoke countertransference enactment. It is demonstrated how this may be solved in the therapeutic process and facilitate integration of traumatic experiences and loss.

ZusammenfassungSeit 2 Jahrzehnten engagieren sich rund 400 Homöopathinnen und Homöopathen im Rahmen der Homöopathen ohne Grenzen e. V. (HOG). Getreu der Leitidee „Hilfe zur Selbsthilfe durch Homöopathie“ leisten sie entweder humanitäre Soforthilfe, wo es sinnvoll ist, oder verbreiten die Lehre der Homöopathie durch gezielte und lokal angepasste Ausbildungsmaßnahmen. In diesem Beitrag soll ein Einblick in die Arbeit bei HOG gegeben werden. Ruanda, ein Land, in dem noch immer mit den traumatischen Folgen des Genozids im Jahr 1994 gerungen wird, ist Ziel einer Reise der Homöopathen ohne Grenzen.

Relationships of Childhood Adverse Experiences With Mental Health and Quality of Life at Treatment Start for Adult Refugees Traumatized by Pre-Flight Experiences of War and Human Rights Violations

Adverse and potentially traumatic experiences (PTEs) in childhood were examined among 54 adult refugee patients with pre-flight PTEs of war and human rights violations (HRVs) and related to mental health and quality of life at treatment start. Extent of childhood PTEs was more strongly related to mental health and quality of life than the extent of war and HRV experiences. Childhood PTEs were significantly related to arousal and avoidance symptoms of posttraumatic stress disorder (PTSD) and to quality of life, whereas pre-flight war and HRV experiences were significantly related to reexperiencing symptoms of PTSD only. Within childhood adversities, experiences of family violence and external violence, but not of loss and illness, were significantly related to increased mental health symptoms and reduced quality of life. These results point to the importance of taking childhood adverse experiences into account in research and treatment planning for adult refugees with war and HRVs trauma.
