Diaphragm Disease of the Small Intestine

Abstract: Diaphragm disease of small intestine usually presents with nonspecific clinical features. Radiological investigations often fail to differentiate it from small intestinal tumors and inflammatory bowel disease. It is therefore diagnosed on final histology after surgical resection. We hereby report an interesting case of a suspected small bowel tumor later diagnosed as diaphragm disease on histology.

“…This made surgery mandatory, as florid locoregional lymphadenopathy has not been previously described in diaphragm disease 6 11. There has been one previous report of diaphragm disease being misdiagnosed with malignancy, however, in this patient there was ongoing treatment for malignancy and imaging showed only minor mesenteric lymph node enlargement 12. The diagnosis of diaphragm disease largely rests on a history of NSAIDs use, anaemia, symptoms of small bowel obstruction and the finding of small bowel strictures on imaging.…”

“…These rings differ from the membranes by their greater thickness. The clinical features can be iron deficiency anemia, intestinal obstruction, a change in bowel habits or acute abdomen secondary to perforation (1,2). It is usually associated with a sustained use of high doses of non-steroidal anti-inflammatory drugs (1-3), but this patient denied any use of these drugs.…”

“…The diagnosis is sometimes difficult due to the fact that the usual imaging tests do not detect the alterations produced by this disease (1,3). It usually requires the use of a capsule endoscopy or enteroscopy with biopsies (1,3). The differential diagnosis is broad, such as infections, lymphoma and systemic diseases, among others.…”

“…Therapy with long-term nonsteroidal anti-inflammatory drugs (NSAIDs) may play a role in the development of small bowel alterations (4). Malignant tumours of the small bowel include adenocarcinoma, carcinoid, lymphoma, leiomyosarcoma, and GIST (5).…”
