Diagnostik und Therapie von Chlamydia-trachomatis-Infektionen

Abstract: Nucleic acid amplification tests are now considered the method of choice to detect Chlamydia trachomatis. These assays have highest sensitivity and also high specificity, comparable to culture. First-void urine is the preferred specimen for urogenital infections of males, whereas vaginal and cervical swabs are at least as effective for testing female lower genital tract infections. Chlamydia point-of-care tests may produce results rapidly without special equipment but lack diagnostic accuracy and thus are unsu… Show more

“… The most important reason for treatment failure is inadequate medicine or dosage and insufficient treatment course, which all tend to induce the drug resistance and persistent infection. Therefore, appropriate education, standard, and enough-course therapy[ 8 33 34 35 ] should be given to diagnosed patients (medium quality of evidence, strong recommendation) We can change the medicines[ 4 ] or prolong the therapy course[ 4 5 34 35 36 37 ] for those therapy-resistant patients. For example, we can use doxycycline or clarithromycin for 14 to 21 days.…”

“…Unfortunately, in some countries azithromycin is not permitted to be administered in pregnancy, and therefore erythromycin or amoxicillin is regarded as an option. 269,272 In countries, where josamycin is available, it is regarded as an alternative drug with a well-tolerated regimen. 243 In the first trimester, it is only recommended under strict conditions.…”

“…Many of them are designed as duplex assays analysing both pathogens. 18 The recently developed PelvoCheck CT/NG test kit from Greiner-Bio-One GmbH also simultaneously analyses CT and NG and can be used for both urine and swabs. To evaluate the performance of the PelvoCheck CT/NG test in FVU, we examined urine samples obtained during Chlamydia screening by both PelvoCheck CT/NG and Roche COBAS TaqMan CT Test V.2.0 as a comparative NAAT, as well as FVU specimens pretested for NG in another laboratory by PelvoCheck CT/NG and an in-house PCR test based on porA gene sequences.…”
