Diagnostik und operative Therapie beim Insulinom - Erfahrungen bei 40 Patienten

Abstract: After positive biochemical and fasting tests and exclusion of diffuse abdominal metastases by transabdominal ultrasound, all patients should without further preoperative investigations undergo surgical excision of the insulinoma after bidigital palpation of the pancreas and IOUS. But if laparoscopic excision is planned, endoscopic ultrasound should be undertaken preoperatively.

“…MEN1 insulinoma is usually diagnosed between the second and third decade of life; in the present study the median age at diagnosis was 27 years. This is about one decade earlier than sporadic insulinoma [38,39] and MEN1-ZES [40,41]. According to previous reports [1,4,6,17,18,20,22], insulinoma is the first manifestation of MEN1 in about 10% of patients, which is in line with the 8% rate in the present series.…”

“…The average sensitivity of transabdominal ultrasonography ranges from 9.6% to 33%. [678] It is an economical, convenient, and noninvasive diagnostic modality and, therefore, still used as a routine method for clinical examination. Although contrast-enhanced ultrasonography with SF 6 is reported to have a much higher sensitivity (92.31%) than the conventional technique,[9] it is not widely used.…”

“…Enhanced computer tomography (CT) has a relatively high sensitivity (33%–60%) for the diagnosis of insulinomas. [67810] Although CT imaging findings can help exclude metastatic diseases, tumors <1 cm are not easily detected by CT. Arteriography performed in conjunction with CT can detect lesions as small as 1.0 cm × 0.8 cm. [11] Recent advances in CT equipment have enabled the detection of insulinomas with greater efficacy.…”
