Objective: To identify the main nursing diagnoses (NSs) in older adult patients under hemodialysis treatment. Method: Exploratory research using case studies in data collection performed by interview and physical examination of older adults, in the fi rst semester of 2016. Were included twenty-eight older adults undergoing chronic hemodialysis treatment who met the selection criteria. The analysis followed two steps (RISNER, 1990): Phase I -Data analysis and synthesis; and Phase IIEstablishment of nursing diagnoses using the taxonomy of NANDA-I (2015). Results: The total of NSs was 110, averaging 3.9 per patient. It was listed seven different NSs, and both the Risk of infection and the Volume of excessive liquids appeared on all patients (28; 100%), and risk of electrolyte imbalance, in 26 (96.8%) older adults, being considered as main NSs. Conclusion: Such results can help systematize the care of older people who are undergoing hemodialysis treatment. RESUMO Objetivo: Identifi car os principais diagnósticos de enfermagem (DEs) em pacientes idosos em tratamento hemodialítico. Método: Pesquisa exploratória utilizando estudos de casos na coleta de dados realizada por entrevista e exame físico dos idosos, no primeiro semestre de 2016. Foram incluídos 28 idosos em tratamento crônico por hemodiálise que atenderam aos critérios de seleção. A análise seguiu duas etapas (RISNER, 1990): Fase I -Análise e síntese dos dados; e Fase II -Estabelecimento dos diagnósticos de enfermagem utilizando a taxonomia da NANDA-I (2015). Resultados: O total de DEs foi de 110, com média de 3,9 por paciente. Foram elencados sete DEs diferentes, sendo que tanto o Risco de infecção quanto o Volume de líquidos excessivo apareceram em todos os pacientes (28; 100%), e Risco de desequilíbrio eletrolítico, em 26 (96,8%) idosos, sendo considerados como principais DEs. Conclusão: Tais resultados podem colaborar na sistematização da assistência do idoso em tratamento hemodialítico. RESUMO: O objetivo deste estudo foi identificar a frequência de diagnósticos de enfermagem e cuidados relacionados à inserção e manutenção de sonda nasoenteral. Coorte prospectiva de adultos de uma emergência, em hospital universitário do sul do Brasil, entre abril e julho de 2015. Os pacientes foram acompanhados da indicação ao uso da sonda nasoenteral. Prontuários foram revisados para identificar diagnósticos de enfermagem e cuidados nas 24 horas após a inserção. Das 150 inserções de sonda, 20,7% (n=31) tiveram algum diagnóstico de enfermagem relacionado. "Nutrição desequilibrada: menos que as necessidades corporais" foi o mais frequente (n=22, 71%). Em 64 (42,7%) do total de inserções houve prescrição de pelo menos um cuidado. "Implementar cuidados com sonda" foi prescrito 41 vezes de forma isolada ou combinada. Houve maior proporção de cuidados quando havia diagnósticos de enfermagem implementados (p<0,001). Observa-se que, apesar da utilização da sonda nasoenteral exigir diferentes cuidados, a implementação de diagnósticos de enfermagem e cuidados em emergência ainda são baixas. IMPLEMENTAÇÃO DE DIAGNÓSTICOS E CUIDADOS DE ENFERMAGEM PÓS IMPLEMENTATION OF NURSING DIAGNOSES AND CARE AFTER NASOENTERAL TUBE PLACEMENT IN AN EMERGENCY SERVICEABSTRACT: The objective of the present study was to identify the frequency of nursing diagnoses and care regarding the placement and maintenance of nasoenteral tubes. A prospective cohort study was carried out with adults in the emergency unit of a teaching hospital in Southern Brazil from April to July 2015. The patients were followed up from the indication to the use of nasoenteral tubes. Medical records were revised to identify nursing diagnoses and care within 24 hours after the procedure. Of the 150 tube placement procedures, 20.7% (n=31) had some related nursing diagnosis. "Imbalanced nutrition: less than the body requirements" was the most common diagnosis (n=22, 71%). In 64 (42.7%) of the procedures, there was prescription of at least one care. "Implementing care with NET" was prescribed 41 times, isolated or combined. A greater proportion of care was found when there were implemented nursing diagnoses (p<0.001). Although the use of nasoenteral tubes demands special care, the implementation of nursing diagnoses and care in emergency services is still low. Objective: to identify nursing diagnoses, their defining characteristics and related factors, according to the NANDA-International diagnostic classification (2012-2014). Methods: a quantitative study that used as a sample 134 medical records of patients admitted to a university hospital. The analysis was based on descriptive statistics: frequency and percentage. Results: the most prevalent nursing diagnoses are in the areas of coping/stress tolerance, safety/protection and comfort. The following are the diagnoses of: risk of infection (93.8%), impaired skin integrity risk (60.4%) and anxiety (60.4%). There was a significant variation in the interpretation of the same diagnosis in relation to the defining characteristics and related factors identified. Conclusion: diagnoses presented in the research are among the most frequent in hospitalized patients with clinical intercurrences. Descriptors: Nursing Diagnosis; Nursing Records; Nursing. Destacam-se os diagnósticos de: Risco de infecção (93,8%), Risco de integridade da pele prejudicada (60,4%) e Ansiedade (60,4%). Houve significativa variação de interpretação de um mesmo diagnóstico em relação às características definidoras e fatores relacionados identificados. Conclusão: diagnósticos apresentados na pesquisa estão entre os mais frequentes em pacientes internados com intercorrências clínicas.
