Aim: To analyse trends in HIV testing, serial HIV prevalence and HIV incidence among people who underwent voluntary testing in a Center for AIDS Prevention in Valencia, Spain. Methods: Open cohort study including all subjects who went to the Center for AIDS Prevention from 1988 to 2003. Information on sociodemographic variables and HIV test results was collected. Serial prevalence and incidence rates were calculated, and joinpoint regression was used to identify changes in trends over time.Results: 21 241 subjects were analysed; 67% men, 27% injecting drug users (IDUs), 43% heterosexuals and 13% men who have sex with men (MSM). From 1988 to 1990, IDUs accounted for 57% of clinic attenders, decreasing to 14% by 1997-2003, accompanied by an increase in heterosexuals. Overall, HIV prevalence for the whole period was 15%, dropping from 35% to ,10% after 1999 and to 3% by 2003, when HIV prevalence was 26% in IDUs, 6% in MSM and 2% in heterosexuals. Total HIV incidence was 2.5%. From 1988 to 1990, HIV incidence ranged from 6% to 8%, and a gradual and progressive decline observed from 1990 onwards. From 1995 onwards, HIV incidence was ,2%. The highest incidence rate is seen in IDUs, 7-12% in the first period and 4-5% at the end. Among MSM, a change in the decreasing trend is seen by 1998, and increases in incidence are detected by 2002-3. Conclusions: Serial HIV prevalence has markedly decreased from 1988 in all transmission categories, although it is still high. With regard to HIV incidence, the drop has been marked too, although a worrying increase, that requires further follow-up, has been detected in MSM in the past 2 years.

The HIV/AIDS Reporting System based on reporting of newly diagnosed HIV infections is feasible, since it has been useful in achieving the objectives of epidemiological HIV infection surveillance. It also provides more accurate information than does the AIDS Registry, which can be used to describe recent patterns of HIV transmission. The completeness of the new reporting system may be enhanced by including the diagnosis of HIV infection among the diseases of mandatory notification.

Impacto del Targa con y sin inhibidores de proteasas en pacientes con VIH del Hospital Nacional Arzobispo Loayza

Se determina el impacto del TARGA con y sin inhibidores de proteasa en pacientes con VIH del Hospital Nacional Arzobispo Loayza (periodo 2004- 2018). Mediante estudio analítico, observacional, longitudinal, de cohorte, retrospectivo y correlacional, participaron 359 pacientes entre 18 y 29 años de edad, 249 recibieron TARGA sin inhibidores de proteasa y 110 TARGA con inhibidores de proteasa. El 82,2% masculino y 17,8% femeninas con edad promedio de 26,2 años. Observándose incrementos medio de linfocitos TCD4+ con ambos tratamientos, no existiendo diferencia significativa entre los niveles de linfocitos al inicio de los tratamientos (p=0,135); en los controles sucesivos (6, 12 y 18 meses) existe diferencia estadísticamente significativa entre los grupos (p= 0,000, 0,000, 0,006), y en los controles (24 y 30 meses) no existe diferencia significativa (p=0,005 y 0,040) con un nivel de confianza del 95%. Se observó diferencia significativa entre los niveles de carga viral al inicio de los tratamientos p=0,000; en los controles correspondientes a los 6, 12, 18 y 30 (p=0,734, 0,757, 0,475 y 0,541) indican que no existe diferencia significativa entre los niveles de carga viral y el TARGA utilizado. Observándose que ambos tratamientos favorecen la respuesta inmunovirológica en un 67,4% para el TARGA sin inhibidores y en un 32,6% para el TARGA con inhibidores de proteasas, con un nivel de significancia de p=0,035 y un nivel de confianza del 95%. Concluyendo que existe diferencia significativa entre ambos tratamientos, causando un mayor impacto en los niveles inmunovirologicos de los paciente que recibieron TARGA sin inhibidores de proteasa.
