BackgroundRotavirus infection is the most common cause of acute gastroenteritis globally in children under 5 years of age and is responsible for approximately 5% of all child deaths yearly. Rotavirus vaccination is considered an effective public health strategy to prevent infection and reduce the severity of disease. Multi-centre country trials on rotavirus vaccines demonstrated efficacy rates of more than 85% in developed countries but only about 65% in developing nations. Rotavirus vaccination was introduced into the Kenya Expanded Programme on Immunization (KEPI) in 2014. The objective of our study was to determine the prevalence of rotavirus infection, severity of acute diarrhoea and to determine the rotavirus vaccination status among children aged 3–24 months presenting with acute diarrhoea at Kenyatta National Hospital after introduction of rotavirus vaccine in Kenya.MethodsA total of 365 children aged 3–24 months presenting with acute diarrhoea at KNH were recruited from August 2016 to April 2017. Data on rotavirus vaccination status, nutritional status, feeding practices and sociodemographic characteristics were obtained and a full clinical evaluation of the patients was done. Severity of the gastroenteritis was assessed using the 20 point Vesikari Clinical Severity Scoring System. The children who were admitted were followed up for 7 days using hospital ward registers. Comorbid conditions were established from patient’s clinical records and physical examination. Stool specimens from study participants were tested for rotavirus using a commercially available enzyme linked immunosorbent immunoassay kit- ProSpecT Rotavirus Microplate Assay.ResultsMajority of the children (96.7%) had received rotavirus vaccinations. The overall rotavirus prevalence was 14.5% and was higher among 17–24 months at 19.5%. The prevalence somewhat differed by gender, nutritional status, exclusive breastfeeding status, age and education level of mother/caregiver. Overall, a half of the children had severe acute diarrhoea and there were some differences in severity by child/mother characteristics.ConclusionThere is still burden of rotavirus diarrhoea after introduction of rotavirus vaccine and the prevalence varies by child characteristics.

Rotavirus diarrhea causing gastroenteritis in children under five years is an important issue that urgently needs to be addressed globally. Delay in management of rotavirus diarrhea can be fatal. Diagnostic tool for detecting rotavirus is, therefore, needed. However, until now the gold standard diagnostic tools are expensive, often not available and affordable in health care settings. The aim of the study was to compare the Vesikari clinical severity score of rotavirus-positive with rotavirus-negative in hospitalized children with acute gastroenteritis. Furthermore, the difference of the level of treatment between rotavirus-positive with rotavirus-negative was also evaluated. This was a cross sectional study that using secondary data from medical records of five general teaching hospital in Indonesia. Subjects were children aged <5 years with acute watery diarrhea admitted to the hospital. Statistical analysis used was chi square test, U-Mann Whitney, and Kruskal Wallis. The results showed that the patient with rotavirus positive have higher dehydration (80.2%) compared to rotavirus negative (70%). The severity level of clinical feature was higher in diarrhea due to rotavirus positive than non rotavirus (11.47± 2.89 vs 10.41 ± 2.70; p<0.000). The level of treatment was higher in rotavirus positive. The majority had treatment plan C (47.7%) higher than plan B and A (45.6% and 30.9%; p<0.050). This was opposite with patient with rotavirus negative that majority had treatment in plan A (69.1%) higher than plan B and C (54.4% and 52.3%) (p<0.001). In conclusion, the severity of gastroentrities in children under 5 years using vesikari score are higher in diarrhea due to rotavirus positive than non rotavirus. The treatment level plan C is higher than plan B and A in diarrhea due to rotavirus. This is opposite with non rotavirus majority have treatment in plan A higher than plan B and C.

BACKGROUND:Diarrhea remains the most common infectious disease worldwide. Zinc has been studied extensively recently for its potential effect on prevention, control and treatment of acute diarrhoea.AIM:This study was designed to correlate the level of zinc with the severity of pediatric diarrhoea estimated by Vesikari Scoring System.PATIENTS AND METHODS:The present study included 80 children aged two months to 30 months from those suffering from the acute diarrheal episode and admitted to Pediatric Hospital “Abo El Rish” Cairo University. Serum Zinc level was assessed by a colorimetric method with a spectrophotometer.RESULTS:Zinc deficiency was detected in 45 (56.2%) patient of the studied group Significant negative correlations were found between serum zinc level and severity of dehydration and duration of hospitalization (p < 0.05).CONCLUSION:Zinc level has an essential role in acute pediatric diarrhoea. Zinc therapy should be considered beside Oral rehydration salts (ORS) to achieve maximum impact on diarrheal diseases; clinical trials are recommended to support the zinc supplementation in developing countries.
