Diagnostic value of standardized assays for anti-neutrophil cytoplasmic antibodies in idiopathic systemic vasculitis

Abstract: Anti-neutrophil cytoplasmic antibodies (ANCA) are widely used as diagnostic markers for Wegener's granulomatosis (WG), microscopic polyangiitis (MPA), Churg-Strauss syndrome (CSS) and idiopathic rapidly progressive glomerulonephritis (iRPGN). The objective of this study was to evaluate the diagnostic value of ANCA measurement by the indirect immunofluorescence (IIF) test, and by anti-PR3 and anti-MPO ELISA performed in different locations, in patients with idiopathic small vessel vasculitis. Fourteen centers p… Show more

“…Every patient's serum was tested for the presence of ANCA by immunofluorescence according to the recommendations of the European Vasculitis Study Group (14) using sera diluted 1/16 and ethanol-fixed neutrophils. The immunofluorescence assay was completed by enzyme linked immunosorbent assay (ELISA) for every patient with ANCA detected by the former.…”

“…Similarly, the small number of patients in these 2 case series provided selection bias. Evaluation of larger cohorts from both the European Vasculitis Study Group and the Chapel Hill Group implied that approximately 10 -30% of patients with pauci-immune crescentic glomerulonephritis lack positive ANCAs (21,22). In both the Taiwanese and the Chinese case series, ANCA-negative pauci-immune crescentic glomerulonephritis presented at a much younger age than those with ANCA-positive disease (19,20).…”

“…GPA is primarily associated with PR3-ANCA, while MPA is primarily associated with MPO-ANCA. However, 20 % of patients with GPA or MPA have the alternative ANCA, and at least 10 % of patients are ANCA negative [3][4][5]. In many cases, the ANCA titer will decrease or be 0 with remission induction therapy [4,6,7].…”

“…GPA is primarily associated with proteinase 3 (PR3)-ANCA, while MPA is primarily associated with myeloperoxidase (MPO)-ANCA. However, 20 % of patients with GPA or MPA have the alternative ANCA, and at least 10 % of patients are ANCA negative [3][4][5]. ANCAs play an important role in the pathogenesis of GPA and related vasculitides.…”
