Osteochondral lesion of the talus (OLT) is a common condition associated with ankle injury that brings challenges in the diagnosis and treatment. Symptoms related to this condition are nonspecific including pain, swelling, stiffness, and mechanical symptoms of locking and catching. While the natural history of the OLTs is not well understood, surgical treatment is often required especially in chronic cases and acute cases with displaced articular fragments. Arthroscopic treatment of the OLTs aims to restore ankle joint function and pain relief by the removal of the chondral or osteochondral fragment, debridement and stabilization of cartilage rim and subchondral bone, and stimulate healing of the bone and damaged cartilage. In patients with a large lesion or after a failure of previous bone marrow stimulation, biologic restoration techniques including the use of particulate juvenile cartilage techniques, autogenous chondrocyte implantation, and osteochondral autograft or allograft transplantation may have role. This article summarizes the contemporary concepts in the clinical evaluation and treatment of OLTs with particular emphasis on surgical strategies.

ResumenLas lesiones osteocondrales de la articulación subtalar es una patología infrecuente y de incidencia variable, dado su reporte principalmente como hallazgo en el estudio de dolor crónico de tobillo y ya con cambios degenerativos articulares. La sospecha clínica y el estudio imagenológico dirigido, permiten investigar esas lesiones en estadios iniciales, evitando así el uso de técnicas que no preservan la articulación para su manejo. La artroscopía subtalar es una excelente herramienta tanto diagnóstica como terapéutica para la resolución de dichas lesiones. Dadas las características anatómicas y biomecánicas de la articulación, en estadios iniciales, el manejo mediante sinovectomía y microfracturas es una alternativa con excelentes resultados funcionales.Este trabajo incluye dos casos de lesiones osteocondrales de la faceta posterior de la articulación subtalar manejadas vía artroscópica mediante sinovectomía y microfracturas y su posterior evolución.

ZusammenfassungBildgebende Verfahren spielen in der Diagnostik von Erkrankungen aus dem rheumatischen Formenkreis eine zentrale Rolle. Zahlreiche autoimmun-rheumatische Erkrankungen zeigen relativ spezifische Befallsmuster, so dass die Bildgebung zur Diagnosestellung und Verlaufsbeurteilung beiträgt. Das konventionelle Röntgenbild ist seit Langem nicht mehr die einzige zur Auswahl stehende bildgebende Methode. Da in frühen Erkran- kungsstadien v. a. weichteilige Veränderungen im Vordergrund stehen, sind z. B. die Sonografie und Magnetresonanztomografie der konventionellen Bildgebung hinsichtlich der diagnostischen Genauigkeit überlegen. In diesem Übersichtsartikel soll daher der aktuelle Stand zur bildgebenden Diagnostik rheumatischer Erkrankungen dargestellt sowie ein Ausblick auf derzeitige technische und klinische Entwicklungen gegeben werden. Es werden die konventionelle Radiografie, die Computertomografie, der Ultraschall, nuklearmedizinische Verfahren, die Magnetresonanztomografie und die optische Bildgebung diskutiert.
