Diagnostic value of serum soluble triggering expressed receptor on myeloid cells 1 (sTREM-1) in suspected sepsis: a meta-analysis

Abstract: Background: We aim to synthesize the up-to-date studies to investigate the diagnostic value of serum soluble triggering expressed receptor on myeloid cells 1 (sTREM-1) in suspected sepsis. Results: A total of 19 studies with 2418 patients were finally enrolled in the meta-analysis. The pooled sensitivity was 0.82 (95% CI 0.73 to 0.89), specificity 0.81 (95% CI 0.75 to 0.86), positive likelihood ratio 4.3 (95% CI 3.02 to 6.12), negative likelihood ratio 0.22 (95% CI 0.24 to 0.35), diagnostic odds ratio 20 (95% … Show more

“…The sensitivity of sTREM-1 in periodontitis is comparable to the effects of sTREM-1 on various systemic diseases as indicated in multiple studies. [7][8][9][10] Our study also illustrates the positive correlation between the evaluated biomarkers. "sTREM-1, IL-1β, and MMP-8" are positively correlated with each other achieving statistical significance (p < 0.001) and this substantiates the strong inflammatory profile of this short panel of markers besides validating the underlying inter-connection between these 3 molecules.…”

“…Then the summary positive and negative likelihood ratios and area under the receiver-operating characteristic curve (AUROC) were calculated according to the corresponding formulas. For meta-analysis, the pooled sensitivity and specificity were presented with midas and metannif modules in Stata 16.0 (StataCorp LP) [ 14 ]. The summary diagnostic odds ratios were calculated utilizing a Der-Simonian and Laird random effects model with a corresponding test of heterogeneity.…”

“…Engagement of TREM-1 triggers secretion of IL-8, monocyte chemotactic protein-1, and TNF-α and induces neutrophil degranulation [ 46 ]. In a 2020 meta-analysis of 19 studies involving 2418 patients, the ability of sTREM-1 to diagnose sepsis had a pooled sensitivity and specificity of 0.82 (95% CI, 0.73–0.89) and 0.81 (95% CI, 0.75–0.86), respectively [ 47 ]. In adult populations, sTREM-1 levels in septic patients with a positive culture were significantly higher in nonsurvivors compared to survivors, but they failed to have any value in culture-negative septic patients [ 48 ].…”
