Diagnostic Value of Muscle [11C] PIB-PET in Inclusion Body Myositis

Abstract: Background: The accumulation of multiple-protein aggregates within muscle fibers is a pathological hallmark of sporadic inclusion body myositis (s-IBM) with the presence of inclusion bodies. Amyloid-beta is one of the accumulated proteins in s-IBM. The aim of this study was to elucidate the utility of Pittsburgh compound B-positron emission tomography (PIB-PET) for diagnosing s-IBM. Methods: Nine patients with s-IBM and four patients with idiopathic inflammatory myopathy (IIM) were included. Patients underwent… Show more

“…Amyloid positron emission tomography (amyloid-PET) has been investigated as a diagnostic tool for IBM and differentiates it from other IIMs such as PM ( 160 162 ). These studies have used a relatively small number of patients so far.…”

“…MRI [22][23][24][25][26][27] fast spin-echo T1 imaging has shown prominent muscle atrophy and fat infiltration in a selective distribution with a proximal to distal increasing gradient within a given muscle, and STIR imaging has shown muscle edema often referred to as "inflammation" but whose precise pathologic correlate is unknown. PET imaging with the brain beta-amyloid tracers [ 11 C]PIB-PET 28,29 and [ 18 F]florbetapir 30 for detection of misfolded protein aggregates, noted to have been incorrectly called "beta amyloid", 31 and with the glucose analog 2-deoxy-2-[ 18 F]fluoro-D-glucose (FDG) 32 have also been performed in IBM. Both amyloid and FDG-PET are limited by a lack of specificity for the underlying disease mechanism in IBM.…”

“…Fluorine-18 ( 18 F)-labeled fluorodeoxyglucose PET ( 18 FDG-PET) can identify patterns of muscle involvement, and detect concurrent interstitial lung disease and malignancy in IIM via glucose metabolism ( 17 ). 11 C-labeled Pittsburgh compound B-PET (PIB-PET) ( 18 , 19 ) and 18 F-Flobetapir PET identify amyloid-beta in IBM ( 20 ). The use of PET imaging that utilizes a CD8 T lymphocyte-based ligand shows tremendous potential in preliminary studies in IBM ( 21 ).…”
