Diagnostic value of interleukin-6 and C-reactive protein on early onset bacterial infection in preterm neonates with respiratory distress

Abstract: The combination of IL-6 (0 h) and/or CRP (24 h) is helpful for excluding early onset infection in preterm infants with RD but the poor specificity limits its potential benefit as a diagnostic tool.

“…The association between IL-6 and peripheral infection has been well described ([25-27,30,31] and has been indicated as of value in diagnosis [32-34]. It is important to note that ours was a post-hoc analysis, driven by observed data, and therefore likely to overstate the association.…”

“…In a similar study, PCT and IL-6 were both significant independent predictors of severe sepsis and were superior to CRP 60 . More studies have been published in pediatric patients with suspected sepsis which concluded that IL-6 levels might be helpful for the diagnosis of sepsis 61 - 65 . However, a conclusive study with large sample size is also missing for this patient population.…”

“…The risk of developing bacteraemia from ingested lactobacilli is less than 1 per 1 million users; risk of developing fungaemia (from Saccharomyces Boulardii) is less than1 per 5.6 million users [14-16]. In many studies on infants, C- reactive protein (CRP) and IL-6 have been used to diagnose the early onset of infection [17,18]. CRP is an acute phase protein, blood levels begin to rise to 10 – 1000 fold from 1 ug/ml within 4–6 hours at the onset of an infective or inflammatory process.…”
