Diagnostic testing in fecal incontinence

Abstract: Fecal incontinence is a common problem that disproportionately affects women and the elderly and has a significant impact on the quality of life. Incontinence is often multifactorial. Anorectal manometry, anal endosonography, magnetic resonance imaging, pudendal nerve latency, and electromyography provide morphologic and physiologic assessments of the internal and external anal sphincters, rectal motor and sensory function, rectal compliance, and rectoanal reflexes. This information, in concert, provides clues… Show more

“…It is of most importance for selecting a treatment option, and establishing the actual situation of the anal canal sphincter, not only by the recognition of a muscle tear but also by identifying the location and amount of tissue involved (9). In this regard, anal endosonography, a cheap, easy-to-perform and well-tolerated examination, is very useful (10).…”

“…The coming of anal endosonography to the study of fecal incontinence has probably been the most important improvement in the management of these patients during the last decades. It is of most importance for selecting a treatment option, and establishing the actual situation of the anal canal sphincter, not only by the recognition of a muscle tear but also by identifying the location and amount of tissue involved (9). In this regard, anal endosonography, a cheap, easy-to-perform and well-tolerated examination, is very useful (10).…”

“…Pudendal nerve terminal motor latency test (PNTML) assesses the neuromuscular circuit between the terminal branch of the pudendal nerve and the external anal sphincter by measuring the conduction time between the initial stimulation and the EAS contraction (seen by motor evoked potential curve). Prolonged latency time suggests pudendal neuropathy [76]. However, the test is not sensitive enough to be related with clinical symptoms, manometric findings, and histologic findings [76,80].…”

“…Electromyography (EMG) performed by inserting a needle electrode in the external anal sphincter muscle and levator ani muscle had been used to assess integrity of neuromuscular connection of the muscle [76]. Due to invasiveness, surface EMG had also been used [77].…”
