Diagnostic sensitivity of serum and lumbar CSF bHCG in newly diagnosed CNS germinoma

Abstract: Background Marked elevations of AFP and bHCG in serum or CSF may serve as surrogate diagnostic markers in lieu of histology for primary CNS mixed, malignant germ cell tumors. There is less information on the diagnostic sensitivity of bHCG assays in germinoma. Procedure We report baseline serum and lumbar CSF bHCG values in 58 newly diagnosed, histologically confirmed germinoma patients gathered from 2 prospective clinical trials which required that patients have a normal AFP and bHCG ≤50 mIU/ml in serum and … Show more

“…Pure germinomas should always have normal AFP levels in the serum and lumbar CSF but may have a low level of HCG. Assays of lumbar CSF is imperative for an accurate diagnosis and staging since oncoprotein levels are often much higher than in the serum [12]. Lumbar CSF assays are a more reliable source than ventricular CSF for repeated accessibility and published norms [13].…”

“…39 Elevated serum and CSF AFP and BHCG may serve as markers for germ cell tumours, although they do lack sensitivity. 40 Germinomas may present with normal AFP levels in both serum and CSF, and normal or mildly elevated BHCG levels. 41 The classic MRI finding in CDI is loss of the posterior pituitary two-thirds of cases.…”
