Aim:The Coronavirus disease-2019 , declared as a pandemic by the World Health Organization on March 11, 2020, is a condition caused by Severe acute respiratory syndrome-Coronavirus-2, and the number of cases is increasing day by day. The aim of this study is to investigate the differences in systemic inflammatory indices of newborn babies born to COVID-19 polymerase chain reaction (PCR)-positive mothers, who constitute a sensitive population during the COVID-19 pandemic period, compared to the normal population. Materials and Methods:Between March 2019 and November 2021, in Ankara City Hospital, newborns who were born at ≥37 weeks of gestation to COVID-19 PCR positive mothers in the two weeks before birth and whose COVID-19 PCR tests were negative and were given usual care with mothers, basal hematological parameters were compared by taking healthy newborns born to COVID-19 PCR negative mothers at ≥37 weeks of gestation in the control group. Results:The rate of cesarean delivery was higher in the group (n=86) of babies of COVID-19 PCR positive mothers (p<0.05). Considering the hemogram parameters, total white blood cell, neutrophil, and hemoglobin/hematocrit counts were lower in the control group (n=94), and platelet/ plateletcrit values were higher (p<0.05). Neutrophil lymphocyte ratio and systemic immune-inflammation index were statistically significantly higher in the infants of COVID-19 PCR-positive mothers, neutrophilia and neutrophil lymphocyte ratio was determined as independent predictive variables in logistic regression analysis (p=0.048 and p=0.011).Results: Vertical viral transmission was not observed in babies born to COVID-19 PCR positive mothers. Compared to the control group, it was thought that the high neutrophil lymphocyte ratio and neutrophilia in babies born to COVID-19 PCR positive mothers might be due to maternal cytokine release. Our study will shed light on further research for the elaboration of this situation and longer follow-up.

Giriş: Doğumda umbilikal kord kan gazı değerleri yenidoğanın asit-baz dengesini gösteren en iyi yöntemdir. Term ve geç preterm bebeklerde kan gazı değerleri yenidoğanın klinik sonuçları hakkında bilgi verirken aşırı prematürelerde bu değerlerin kullanımı açık değildir. Bu çalışmada <29 gebelik haftasında (GH) doğan preterm bebeklerde umbilikal kord kan gazı değerlerinin mortalite ile ilişkisinin değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Çalışmaya GH <29 hafta olan bebekler dahil edilmiştir. Çalışmamız retrospektif olarak gerçekleştirilmiştir. Çalışmaya dahil edilen hastalarda kaybedilen ve yaşayan gruplarda umbilikal kord gazı (pH: potansiyel hidrojen, pCO 2 : parsiyel karbondioksit basıncı, HCO 3 : bikarbonat, BE: baz fazlası) değerleri, demografik özellikler ve klinik sonuçlar karşılaştırıldı. Bulgular: Kaybedilen 54 ve yaşayan 292 hasta olmak üzere çalışmaya toplam 346 hasta dahil edildi. Sonuçlarımıza göre <29 GH olan prematürelerde mortalite oranı %15,6 (54/346) olarak tespit edildi. Yaşamını kaybeden grupta yaşayanlara göre GH ve doğum ağırlığı daha düşük bulundu (sırasıyla, p<0,001, p<0,001). Aynı zamanda yaşayanlara göre pH, HCO 3 ve BE değerleri de kaybedilenler grubunda daha düşüktü (sırasıyla; p<0,001, p<0,001, p<0,001). Gruplar arasında pCO 2 değerleri açısından sonuçlar benzer bulundu (p=0,270). Mortalite için eşik pH değeri ≤7,18 [area under curve (AUC): 0,627], eşik HCO 3 değeri ≤19,3 mmol/L (AUC: 0,950) ve eşik BE değeri ≤-8,1 mmol/L (AUC: 0,969) olarak tespit edildi. Sonuç: GH <29 doğan prematüre bebeklerde umbilikal kord kan gazında mortalite göstergesi için en değerli parametreler HCO 3 ve BE'dir.

Enteral plasma feeding improves gut function and immunity in piglets after birth asphyxia

Introduction Birth asphyxia may negatively affect gut function and immunity in newborns. Conversely, immunomodulatory milk diets may protect the gut and immune system against damage caused by asphyxia. Using caesarean-derived pigs as models, we hypothesised that enteral feeding with plasma improves gut and immune functions in asphyxiated newborns. Methods Near-term pig fetuses (98% gestation,) were delivered by caesarean section after 8 min umbilical cord occlusion, leading to transient birth asphyxia (ASP, n = 75) and compared with non-occluded controls (CON, n = 69). Piglets were further randomised to supplementation with/without porcine plasma (plasma, PLA/vehicle, VEH), into bovine colostrum (first 24 h) or formula (until 72 h). Results Compared with CON, ASP piglets took longer to achieve stable respiration and showed reduced blood pH, weight gain and survival. Independent of asphyxia, plasma supplementation reduced gut haemorrhagic lesions, permeability and inflammatory cytokines together with improved villous morphology and brush-border enzyme activities. Asphyxia reduced blood cytokine responses to ex vivo bacterial stimulation, whereas plasma supplementation ameliorated this effect. Conclusion Dietary plasma supplementation improves survival, gut functions and immunity in both normal and asphyxiated newborns. The components in plasma that mediate gut-protective effects in piglets remain to be identified, but may benefit also birth-compromised newborn infants. Impact Complicated deliveries leading to birth asphyxia, may negatively affect gut, liver and immune adaptation in the first days after birth. Using a model of birth asphyxia in caesarean-derived piglets, we show that enteral feeding with maternal plasma exerts gut maturational and immunomodulatory effects in both control and asphyxiated animals in the first days of life. The mechanisms behind the gut-protective effects of plasma are unknown, but plasma components hold potential for new oral therapies for compromised newborn infants as well as piglets.
