Diagnostic, Prognostic, and Predictive Role of Ki67 Proliferative Index in Neuroendocrine and Endocrine Neoplasms: Past, Present, and Future

Abstract: The introduction of Ki67 immunohistochemistry in the work-up of neuroendocrine neoplasms (NENs) has opened a new approach for their diagnosis and prognostic evaluation. Since the first demonstration of the prognostic role of Ki67 proliferative index in pancreatic NENs in 1996, several studies have been performed to explore its prognostic, diagnostic, and predictive role in other neuroendocrine and endocrine neoplasms. A large amount of information is now available and published results globally indicate that K… Show more

“…Moreover, the expression levels were notably higher in metastatic renal cell carcinoma tissues compared to primary ones. Additionally, it has been noted that higher levels of Ki-67 expression correlate with poorer prognoses for patients [23]. PFN1 binds to various proteins in normal physiological activities and can connect extracellular signaling with the intracellular actin skeleton, including important physiological activities such as membrane transport, cell survival and proliferation, and transcription [24].…”

“…The perceived advantages and disadvantages of each counting method are summarised in Table 2. To date, manual counting is considered to be the gold standard method to count the Ki67 proliferation index in neuroendocrine neoplasms 5,10,11 . It is highly accurate, readily available, permitting inclusion/exclusion of specific areas and allowing case‐based adjustment of positive threshold.…”

“…This relationship has been recognized by the World Health Organization (WHO), which in its 5th edition classification, proposed the term “carcinoid tumors with elevated mitotic counts and/or Ki67 proliferation rates” to describe tumors with high Ki67 indices 11 . Numerous studies have recommended the utilization of Ki67 immunohistochemistry for diagnostic testing of bronchial or lung biopsies 12 making Ki67 a critical indicator for tumor diagnosis and pathological grading. In clinical practice, patients with a high Ki67 index are usually associated with a poor prognosis 13 , 14 .…”
