Automated breast (AB) ultrasonography (US) scanners have recently been brought to market for breast imaging. AB US devices use mechanically driven wide linear-array transducers that can image whole-breast US volumes in three dimensions. AB US is proposed for screening as a supplemental modality to mammography in women with dense breasts and overcomes important limitations of whole-breast US using handheld devices, such as operator dependence and limited reproducibility. A literature review of supplemental whole-breast US for screening was performed, which showed that both AB US and handheld US allow detection of mammographically negative early-stage invasive breast cancers but also increase the false-positive recall rate. Technicians with limited training can perform AB US; nevertheless, there is a learning curve for acquiring optimal images. Proper acquisition technique may allow avoidance of common artifacts that could impair interpretation of AB US results. Regardless, interpretation of AB US results can be challenging. This article reviews the US appearance of common benign and malignant lesions and presents examples of false-positive and false-negative AB US results. In situ breast cancers are rarely detected with supplemental whole-breast US. The most discriminating feature that separates AB US from handheld US is the retraction phenomenon on coronal reformatted images. The retraction phenomenon is rarely seen with benign findings but accompanies almost all breast cancers. In conclusion, women with dense breasts may benefit from supplemental AB US examinations. Understanding the pitfalls in acquisition technique and lesion interpretation, both of which can lead to false-positive recalls, might reduce the potential harm of performing supplemental AB US. Background Women with invasive breast cancer are at higher risk of contralateral synchronous cancer. This study aimed at determining if contralateral breast ultrasound (CBUS) examination should be routinely performed in the preoperative evaluation of breast cancer patients. Methods This is a retrospective study of preoperative CBUS examinations performed between January 2012 and April 2015. The charts of patients presenting for biopsy of a Breast Imaging Reporting and Data System (BIRADS) 5 lesion and who had undergone a concomitant contralateral breast US examination were reviewed. Index tumor, lymph node status, American College of Radiology (ACR) breast density on mammogram, total scanning time, and results of CBUS were recorded. Results Of the 3007 patients who underwent breast biopsies during the study period, 360 patients met the inclusion criteria. Index mass size was 19 ± 10 mm. CBUS examination led to 76 biopsies, of which 12 were positive in 11 patients. Detection rate for mammographically occult contralateral invasive cancers was 3.1% (11/360). Contralateral lesion size was 13 ± 10 mm. Breast density was rated ACR C/D for nine women and ACR B for two. In the ACR C/D subgroup (82%), the contralateral cancer detection rate was 4.1%. Average additional scanning time spent required to perform CBUS examination was 3.1 ± 4.9 min. Patients diagnosed with contralateral invasive breast cancer underwent surgery and/or chemotherapy. The treatment strategy was changed in all 11 patients after the detection of a second primary cancer. Conclusion Preoperative CBUS is effective and most beneficial with women presenting ACR C/D breast density. Given its impact on decreasing future morbidity, its routine use should be considered to improve quality healthcare for women diagnosed with breast cancer.

On the background of new research results in screening of breast cancer, together with the expectation of the participants in a screening for breast cancer, the conventional mammography requires supplementation by means of tomosynthesis or additive ultrasound. Alternatively, ultrasound now seems to be an independent method of early detection of breast cancer because of its superior sensitivity, especially in the case of aggressive mammary carcinoma types. The MRI remains at present still a preferred method in high-risk cases and as an additive examination in case of insufficient presentation of the glandular tissue by conventional methods. MRI is also preoperatively valuable for more accurately measuring the extent of multifocal carcinomas.
