Diagnostic delay of more than 6 months contributes to poor radiographic and functional outcome in psoriatic arthritis

Abstract: Objectives(1) To investigate the demographic and clinical characteristics contributing to the delay from symptom onset to the first visit to a rheumatologist; (2) to compare clinical, radiographic and patient-reported outcome measures of those who saw a rheumatologist early in their disease course with those who were diagnosed later.MethodsAll psoriatic arthritis (PsA) patients, fulfilling CASPAR criteria, with an average disease duration of >10 years were invited for detailed clinical evaluation. The total… Show more

“…There is some evidence to suggest that earlier treatment in the disease course is associated with better outcomes [26][27][28] and, conversely, when diagnosis is delayed, clinical outcomes are negatively affected [29,30]. In AS, a study of 334 patients demonstrated that a delay of more than 10 years before starting anti-TNF therapy resulted in an increase in the likelihood of structural damage, compared with those who had a delay in treatment of at most 10 years [31].…”
