Diagnostic and therapeutic role of endoscopic retrograde pancreatography in the management of traumatic pancreatic duct injury patients: Single center experience for 34 years

Abstract: Endoscopic retrograde pancreatography helps clinicians choose a treatment modality for major pancreatic duct injury since it provides information about the precise condition of the major pancreatic duct injury. Endoscopic retrograde pancreatography with transpapillary pancreatic stenting also shows promise as a substitute for laparotomy or pancreatic resection in selected patients.

“…10 Notably, data are based on case series with variable outcome, but endoscopic management has gained both popularity and success, even for main duct disruptions. 10,27,[66][67][68][69][70] Specific endoscopy-based scoring systems for pancreatic duct disruption after blunt trauma have been proposed in a small series from Kanagawa, Japan 71 and a later modified version from Cape Town, South Africa. 67 These scores are quite detailed, with 4-5 categories and several subcategories, thus questioning the robustness of each subcategory.…”

“…When deep parenchymal injury is present (AAST-OIS grade ≥II and JAST type III), enhanced CT is insufficient for accurate identification of pancreatic duct injury [10] . If ductal injury is suspected, ERCP should be planned [4], [15], [16] . Magnetic resonance cholangiopancreatography is also available to confirm the continuity of the pancreatic duct; however, it is not able to detect leakage from the pancreatic duct.…”
