Diagnostic and therapeutic management in gastroesophageal reflux disease: consensus of the Polish Society of Gastroenterology

Abstract: The following paper was developed by an expert group appointed by the Polish Society of Gastroenterology with the goal to update and systematize the knowledge about the diagnosis and treatment of gastroesophageal reflux disease (GERD). Based on the previously published guidelines of international societies, expert consensuses, and recently published good quality data, we formulated 74 statements regarding the definition, diagnosis and treatment of GERD and assessed the level of acceptance of these statements a… Show more

“…The Mediterranean diet is also beneficial for patients with gastroesophageal reflux disease. 65 Thus, following the Mediterranean diet provides numerous health -promoting effects, which are very well documented. Therefore, the European Society of Cardiology recommends adopting the Mediterranean or similar diet to lower the risk of CVD (class of recommendation, I; level of evidence, A).…”

“…Surgical treatment is not well established in patients with extraesophageal symptoms and is not recommended in these cases (58,100) . Numerous studies have examined therapeutic options when clinical versus surgical treatment is in question (99)(100)(101)(102)(103)(104) having observed that the frequency of endoscopic fundoplication has decreased in recent years (105) .…”

“…Unlike PPIs, P-CAB is a new type of highly efficient and selective gastric H+/K+-ATPase inhibitor that does not require activation in a strong acid environment to function. The greatest advantage of tegoprazan, a novel P-CAB, is its ability to bind to the K+ binding site of H+/K+-ATPase in a competitive and reversible manner without acid activation and without any conversion, exhibiting faster gastric acid inhibition than PPIs and almost complete inhibition of gastric acid secretion [ 9 , 14 ]. In addition, P-CAB-induced gastric acid inhibition is not affected by the ab initio synthesis of proton pumps in gastric lining cells, thus enabling faster maximal inhibition and longer duration of action, making P-CAB a novel, highly selective and efficient acid inhibitor [ 15 , 16 ].…”
