Diagnostic Added-Value of Serum CA-125 on the IOTA Simple Rules and Derivation of Practical Combined Prediction Models (IOTA SR X CA-125)

Abstract: Background: This study aimed to evaluate the diagnostic added-value of serum CA-125 to the International Ovarian Tumor Analysis (IOTA) Simple Rules in order to facilitate differentiation between malignant and benign ovarian tumors before surgery. Methods: A secondary analysis of a cross-sectional cohort of women scheduled for surgery in Maharaj Nakorn Chiang Mai Hospital between April 2010 and March 2018 was carried out. Demographic and clinical data were prospectively collected. Histopathologic diagnosis was … Show more

“…Ultrasonography, especially vaginal ultrasonography, is currently the first choice for diagnosing gynecological diseases, 11 and it can clearly show the internal structure of a lesion and its anatomical relationship with the peripheral tissues. However, the accuracy of diagnosis varies greatly based on the experience of the sonographer.…”

“…Besides, other imaging or ultrasound models have been proposed to address these masses, such as MRI, contrast-enhanced ultrasound (CEUS), the Risk of Malignancy Index (RMI), the Risk of Malignancy Algorithm (ROMA), the Assessment of Different NEoplasias in the adneXa (ADNEX) model [27][28][29][30] . A previous study [31] also exploited a logistic regression model based on SR and CA125 and validated the value. While all methods the above solved the problem of inconclusive masses and improved the diagnostic e ciency to a certain extent, it was not easy to execute.…”

“…We hypothesize that diagnostic models created by adding biomarkers of tumor antigen type offer increased but still limited accuracy and, therefore, combining different categories of biomarkers, e.g., CA125 with inflammatory markers, antibodies and age would be the right way to discover maximally effective tools for OC diagnosis. Future studies could evaluate the utility of combining our diagnostic models with sonographic criteria [ 64 ] or body mass index [ 65 ].…”
