Diagnostic accuracy of vesical imaging-reporting and data system (VI-RADS) in suspected muscle invasive bladder cancer: A systematic review and diagnostic meta-analysis

“…In a previous study, our group showed that a VI-RADS of 3 would be the optimal cutoff for the detection of MIBC with a pooled sensitivity and specificity of 89% (95% CI, 87%-91%) and 84% (95% CI, 80%-87%), respectively. [21] Similarly, in this meta-analysis, we demonstrated that VI-RADS at a cutoff of 3 had higher accuracy than that at a cutoff point of 4 for differentiating T1 from T2 urothelial lesions (AUC = 0.93 for VI-RADS ≥3 vs. AUC = 0.75 for VI-RADS ≥4). Although our results show that VI-RADS has a higher pooled specificity at the cutoff point of 4, a VI-RADS ≥3 is preferable in clinical practice for differentiating T2 from T1 bladder tumors because of its higher sensitivity.…”

Our earlier study [3] veri ed that the VI-RADS scoring system has good diagnostic value in predicting the degree of tumor invasion, and several subsequent meta-analyses also further con rmed the ability of the VI-RADS score to reliably diagnose MIBC at a VI-RADS score cutoff of 4, with a speci city above 0.9 [4][5][6][7].
