Introduction: Cerebral palsy is the most common physical disability of childhood. Respiratory problems are the main causes of morbidity and mortality in cerebral palsy. Methods: The study is characterized by a scoping review. The search for articles was carried out in August 2021 in the PubMed, Medline, SciELO, LILACs and Google Schoolar databases, with the keywords "cerebral palsy” and "respiratory". Results: Overall, 1037 articles were found, 10 duplicates were removed and 167 were pre-selected after the analysis of titles and abstracts. Then, 90 were excluded due to lack of appropriateness after reading the full-texts, thus yielding a total of 77 studies. Discussion: Risk of respiratory disease should be screened at least every 12 months based on the following criteria: a hospital admission for respiratory illness in the past 12 months; a Gross Motor Function Classification System level V; a Eating and Drinking Ability Classification System level III–V. The screening aims to lead to early diagnosis and treatment, and consists in actively evaluate the risk factors for emergency department visits and hospital admissions. A Gross Motor Function Classification System level V is the strongest predictor, but dysphagia and seizures are the strongest potentially modifiable factors. Aspiration pneumonia is the main cause of death. The main risk for aspiration are dysphagia; uncontrolled seizures; gastroesophageal reflux disease; and drooling. Other comorbidities should also be actively screened: undernutrition; tone disorders; skeletal malalignment; upper respiratory obstruction; airway clearence impairment; and restrictive lung disease. Conclusion: Respiratory impairments in CP results from a complex multifactorial process influenced by several interrelated pathophysiological factors, directly and indirectly influenced by other common comorbidities in CP. Active and early surveillance, diagnosis and treatment, involving multiple medical specialties and rehabilitation professionals is essential for success in improving the quality of life and reducing morbidity and mortality of these patients.

Objetivo: Identificar características de estudos que validaram conceitos da Classificação Internacional para a Prática de Enfermagem. Método: Revisão integrativa com artigos publicados entre 1996 e 2021, na PubMed, BVS, CINAHL, SCOPUS, Web of Science e SciELO. Incluídos artigos em português, inglês ou espanhol que validaram diagnósticos, resultados ou intervenções de enfermagem. Excluídos revisões de literatura, notas prévias e editoriais. Coletadas características relacionadas ao conceito validado, tipologia de validação, escala e técnica, tipo e seleção da população do estudo. Apresentação dos resultados organizada pela diretriz PRISMA. Resultados: Amostra de 27 artigos. Destacou-se a validação de conceitos relacionados à morte digna, à atenção à mulher no pós-parto e à pediatria. Predominaram validação de conteúdo e clínica; 41% das pesquisas utilizaram o índice de concordância e 26% o índice de validação de conteúdo. Houve a participação de especialistas em 96% das validações. Conclusões: Escala Likert, avaliação por especialistas e índice de concordância são estratégias adotadas nas pesquisas de validação. Para seleção de especialistas, os critérios são modificados e a experiência clínica é o critério, não padronizado, mais utilizado.

PurposeNurses’ clinical reasoning skills regarding impaired swallowing can help prevent patient complications and maintain quality of life. Clinical reasoning skills need content‐validated defining characteristics (DCs). We aimed to validate the content of these DCs for nursing diagnosis “impaired swallowing.”MethodsContent validation of the DCs was performed by 275 dysphagia nursing experts in Japan, using 3 rounds of the Delphi technique and Fehring's Diagnostic Content Validation (DCV) model. Three rounds of questionnaires on 84 DCs were completed via printed mail.FindingsThe valid response rates for each round were as follows: round 1, 90.2%; round 2, 77.8%; and round 3, 71.3%. Of the 84 DCs, 77 that met the consensus criteria were categorized as major (n = 18), minor (n = 45), and excluded (n = 14). There were four minor DCs other than the oral, pharyngeal, and esophageal phases. DCs listed from outside NANDA‐I included 12 major, 16 minor, and 3 excluded characteristics. Of the NANDA‐I DCs, 5 were no consensus and 11 were excluded. The total DCV score for the 63 major and minor DCs was 0.8.ConclusionsOur results recommend the addition of 28 DCs and the exclusion of 11 for the NANDA‐I nursing diagnosis “impaired swallowing” (00103). Major DCs were prominent indicators of impaired swallowing and signs of aspiration or pharyngeal residuals. Minor DCs included not only the three phases but also other signs necessary for a comprehensive understanding of impaired swallowing.Implications for nursing practiceThis validation study strengthens the clinical usefulness of the DCs for impaired swallowing, which can improve nurses’ clinical reasoning skills. Major and minor DCs can increase the awareness of impaired swallowing and enable accurate intervention, thereby preventing patient complications and maintaining quality of life.
