Diagnostic accuracy of sonographic criteria for evaluation of cervical lymphadenopathy.

Abstract: Although ultrasonographic criteria for abnormal nodes are used routinely in the evaluation of cervical lymphadenopathy, the diagnostic accuracy of these criteria in different areas has not been documented. This study evaluated 977 normal cervical nodes from 80 normal subjects and 1419 abnormal cervical nodes from 277 patients with proven cervical lymphadenopathy. The diagnostic accuracy (sensitivity, specificity, positive predictive value, negative predictive value, and accuracy) of size, shape (short axis to … Show more

“…[9][10][11][12][13][14] The accuracy of short axis diameter in our study was 59% and p value of 0.08 using cut-off diameter of 1 cm, whereas in a study by Lyschick et al 7 using a cut-off diameter of 8 mm the accuracy was 65%, seen to be slightly better when smaller sizes were used. The S/L Ratio of ≤0.6 mm for benign nodes had accuracy of 65%, p value of 0.011 was similar to previous studies supporting the round shape for malignant nodes.…”

“…[9][10][11][12][13][14] With short axis diameter cut-off of 10 mm, 20 of 64 lymph nodes were labelled as malignant (38 were actually malignant on HPE). Sensitivity and specificity of 71% and 50% noted respectively with insignificant p-value (0.08), whereas in a study by Lyschick et al 7 using a cut-off diameter of 8 mm the accuracy was 65%, seen to be slightly better when smaller sizes were used.…”

“…The presence of a hyperechoic hilum is one of the important criteria for the normality of parotid lymph nodes (Fig 7). Their short axis should not exceed 5-6 mm in the normal state (6,7). With the application of sensitive power Doppler US, central vessels may be seen in normal parotid lymph nodes.…”

“…1 Lymphoscintigraphy is of limited value in patients with palpable nodes that are clinically suspicious. 2 High-resolution sonography has been used in the evaluation of the neck 3,4 and axillary nodes. 5 Evaluation of groin nodes for metastasis is more difficult than in other sites because of a high incidence of palpable nodes.…”
