Diagnostic accuracy of icteric index to detect abnormal total bilirubin values

Abstract: This study supports the use of the icteric index to identify patients with abnormal total bilirubin values. Considerable economic savings can be achieved if this strategy is applied on a clinical daily basis. Its implementation and report should be discussed and planned by laboratory professionals together with requesting physicians.

“…Thus, this would be at least partly responsible for differences in results. Salinas et al did not clearly discriminate between results on serum samples and plasma samples; they described, however, a small stat subpopulation showing similar characteristics to those of our plasma samples, with a substantial worse sensitivity (77.4%) than in other examined populations 1. This group of samples was measured on a different platform (Dimension RxL Max, Siemens Healthcare Diagnostics), and we may suppose that, being stat samples, plasma was used.…”

“…We read with great interest the article by Salinas et al 1 suggesting the use of the icteric index (II) as a front-line test for the preliminary identification of blood samples with abnormal total bilirubin (TB) concentrations. Following the authors’ suggestion, we were encouraged to experimentally confirm their findings in our own hospital setting.…”

“…Time of exerting effects: The time of icteric index that was measured with a transcutaneous jaundice meter (JM-103, Chengdu Beisida Instruments Co., Ltd.) decreased. 5 Clinical outcomes: Markedly effective: Fading of yellowish pigmentation and obvious decrease in serum bilirubin level. Effective: Basic fading of yellowish pigmentation and moderate decrease in serum bilirubin level.…”

“…There was, however, a great variability between areas. Serum ALT is the recommended screening test for liver disease, ALP is the second choice and bilirubin should be requested only when icterus is suspected [18]. Such an approach would reduce aspartate transaminase (AST)/ALT, tBIL/ALP, LDH/ALT and gamma-glutamyl transferase (GGT)/ALT ratios.…”
