“…The diagnosis of sarcoidosis was made on the basis of the compatible clinical pictures and histologic demonstration of noncaseating granulomas [12, 13] The diagnosis of HP and sarcoidosis needed exclusion of other diseases capable of producing a similar histologic picture: Biopsy tissues were subjected to special stains (acid fast bacilli stain and Gömöri methenamine silver stain) to rule out microorganisms and fungi. IPF patients were evaluated using serial FVC and DLCO measurements.…”

“…A medical centre that performed[10,000 Kveim-Siltzbach tests over 50 years identified a true positive rate of[50% and false negative rate of nearly zero (Iannuzzi et al 2007). This test is now less often used since no commercially available preparation of the reagent exists and the problem that each new Kveim-Siltzbach preparation requires validation in vivo (Parrish and Turner 2009). Furthermore, use of human tissue extracts for clinical purposes presents constraints, including risks of transmitting infections such as Creutzfeldt-Jakob disease even after heating, phenol and irradiation (de Silva and Will 1993).…”

“…It was performed by intradermal injection of Kveim reagent, a validated suspension of human sarcoid tissue, typically sarcoid spleen homogenised in phosphate-buffered saline (Siltzbach 1961;Teirstein 1986). The resultant papule at the injection site was biopsied 4 weeks later and presence of non-caseating granulomas indicated sarcoidosis (Kveim 1941;Chase 1961;Parrish and Turner 2009). In 1967 Siltzbach demonstrated that sarcoid patients worldwide reacted positively to a single batch of Kveim reagent indicating the possibility of a common antigen in the aetiology of sarcoidosis (Siltzbach 1967).…”

“…Sarcoidosis is a chronic multisystem inflammatory granulomatous disease. The etiology of sarcoidosis is unknown, but it is believed to be due to immunodysregulation . Sarcoidosis can be insidious and develop over several years.…”
