Diagnosis of right bundle branch block: a concordance study

Abstract: Background Right bundle branch block is one of the most common electrocardiographic abnormalities. Most cases of right bundle branch block are detected in asymptomatic patients in primary care, so a correct interpretation of electrocardiograms (ECGs) at this level is necessary. The objective of this research is to determine the degree of concordance in the diagnosis of incomplete and complete right bundle branch block between four primary care researchers and a cardiologist. … Show more

“…This knowledge of concordance (or discordance) is important when evaluating the feasibility of new models of care to manage both the clinical load and patients' needs and preferences. For example, a study on the concordance of cardiac right bundle branch block diagnosis between cardiologists and general practitioners had substantial agreement, indicating general practitioners had scope to be more involved [35]. However, a study on the diagnosis of skin conditions between general practitioners and dermatologists had moderate agreement, indicating more training was needed if the model was to change [36].…”

“…On the other hand, the right BBB (RBBB) is a blockage of electrical impulses to the heart's right ventricle and it is one of the most common electrocardiographic abnormalities that is often detected in asymptomatic patients [4] . Although the RBBB has been associated with fewer complications for cardiovascular disease in comparison to LBBB [5] , the complete RBBB (CRBBB) has an impact on patients with other diseases, including acute myocardial infarction, and the appearance of CRBBB in patients with other cardiovascular diagnoses worsen their prognosis [6] , so it is advisable to develop tools for early detection.…”
