Diagnosis of Myeloproliferative Disease by Analysis of the Platelet Volume Distribution

Abstract: Analysis of platelet volume distribution curves was performed on whole blood specimens from patients with myeloproliferative disease, reactive thrombocytosis, and a control group. Estimates of the mean platelet volume and megathrombocyte index were made using either the maximum height or the area under the curve. Also, a lognormal curve was fitted to the data, providing a measure of the dispersion and another estimate of the mean platelet volume. An index expression the breadth of the distribution was derived … Show more

“…Increased MPV and heterogeneity in platelet volume and mass were associated with decreased platelet density in this study. These findings are in agreement with those from early studies using platelet morphology in blood smears or other Hematology analyzers [21][22][23][24]. The increase in platelet volume and the decrease in platelet buoyant density that we observed could be explained by proliferation or dilation of the dense tubular and open canalicular system and reduction of the mitochondria, dense granules, and -granules, as found in ultrastructural studies of platelets from ET patients [25][26][27][28].…”

“…But the values of the six parameters obtained in the controls were independent of sex and age. Other studies have shown that platelet volume is not influenced by sex and age [2,9,20,21]. Our results also show that the incidence of a peak of small elements giving the curve a bimodal aspect, was not significantly dif ferent between CT and PT patients.…”

“…Nevertheless, the anticoagu lant used for blood collection must be the same since it is well known that EDTA in duces a swelling of platelets in the two first hours after blood collection [8,12,21], So the median which takes into account MPV as well as PDW (which is a good index of platelet heterogeneity [3]) seems to be very representative of the whole blood popula tion and deserves to be tested in other pa thologies where platelets are affected.…”

“…Moreover, the available means to differentiate these two entities are not specific enough. Some authors tried to distinguish thrombocytosis in ET from reactive thrombocytosis by using PLT parameters provided by blood analyzers [7, 8]. …”
