Diagnosis of heterotopic bone marrow in the mediastinum using52Fe and positron emission tomography

Abstract: A patient with hereditary spherocytosis was admitted with mediastinal masses on the chest X-ray. 52Fe and positron emission tomography (PET) showed uptake of 52Fe in the masses and established the diagnosis of thoracic extramedullary hematopoiesis.

“…A negative pressure at this site facilitates this phenomenon, Furthermore, physiological extramedullary haematopoiesis has been described at paravertebral sites in the pig (13), possibly as a result of reactivation of quiescent rests of embryonic haematopoiesis. Finally, there is no relationship between the degree of anaemia and the quantity of erythropoiesis in expansion areas (10).…”

“…Indeed, extramedullary haematopoiesis could be prevented by early diagnosis and splenectomy: it has never been described following splenectomy in a case of hereditary spherocytosis. Splenectomy was carried out in 13 patients after the onset of extramedullary haematopoiesis (2,(6)(7)(8)(9)(10)(11). A fatty transformation of the masses has been observed subsequently on follow-Hereditary spherocytosis may present only a limited number of phenotypes by SDS-PAGE.…”
