Diagnosis of anti‐laminin γ‐1 pemphigoid by immunoblot analysis

Abstract: Here we show a novel two-step IB assay using the two recombinant laminin trimers and lam-γ1 chain monomer for the detection of anti-lam-γ1 serum IgG with high sensitivity and specificity. This assay will facilitate the diagnosis and further characterization of this disease.

“…Serologically, PNP is only rarely associated with IgG antibodies against desmoglein 3 and desmoglein 1, which are the main antigenic targets of pemphigus vulgaris and foliaceus respectively, whereas IgG autoantibodies against envoplakin, periplakin and the protease inhibitor, alpha-2-macroglobulin-like antigen-1 are more frequently detected. Other IgG autoantibodies targeting desmocollins, BP180/BP230, and, occasionally, laminin gamma 1 may be also detected in PNP sera [138][139][140][141][142][143]. As a result, PNP requires a specific diagnostic approach, e.g.…”

“…5 Other causes of mucosal erosions and ulcerations include other AIBD, such as laminin c-1 pemphigoid and epidermolysis bullosa acquisita, Stevens-Johnson syndrome, erythema multiforme and erosive lichen planus (LP). 6,7 Of note, cases of oral MMP with clinical and histopathological characteristics of oral LP have been highlighted recently. 8 Moreover, coexistence of lichenoid mucositis with acantholysis is typically found in paraneoplastic pemphigus.…”

“…Then, if neither result was positive, the reaction to the laminin c-1 chain monomer was investigated as a second step. 7 The use of an algorithm based on this two-step IB assay to guide it towards the correct diagnosis when a reaction is observed on the dermal side under indirect immunofluorescence microscopy using salt-split human skin is significant.…”

“…In this study by Solimani et al ., the sensitivity of the serodiagnosis of anti‐laminin γ‐1 pemphigoid was increased to 98.2% in 55 sera by introducing a two‐step IB assay in which the reactivity of the sera to recombinant heterotrimer proteins of laminin‐111 and laminin‐411 was examined as a first step. Then, if neither result was positive, the reaction to the laminin γ‐1 chain monomer was investigated as a second step . The use of an algorithm based on this two‐step IB assay to guide it towards the correct diagnosis when a reaction is observed on the dermal side under indirect immunofluorescence microscopy using salt‐split human skin is significant.…”
