One of the major concerns following cornual ectopic pregnancy, as shown in this case, is uterine rupture in future pregnancies. Uterine rupture has been described at 20 weeks gestation in a woman who had a cornual pregnancy treated by salpingectomy at 24 weeks, and as such it has been suggested by some experts that suturing the uterine wall to reinforce the defective area is advisable. Conversely, term deliveries have been described in patients with laparoscopic treatment of cornual pregnancies without reinforcing sutures. There is general agreement that suturing the uterine wall should be performed in cases where the cornual pregnancy sac extends into the endometrial cavity. Long-term follow-up of patients treated with cornual pregnancies with endometrial involvement may reveal the optimum treatment technique. In addition to surgical technique, patient counselling and appropriate debriefing regarding increasing interpregnancy interval is paramount in reducing maternal and fetal morbidity. Proper documentation in operative notes is vitally important for future reference.

Систематизация и анализ опубликованных данных для уточнения терминологии, факторов риска, мето-дов диагностики и лечебной тактики для внематочной (эктопической) стеночной беременности.Материалы и методы. Поиск информации, опубликованной в электронных библиографических базах данных PubMed и Cochrane Library, по ключевым словам. Ограничения: английский язык, период до 31 октября 2017 года включительно. Для анализа отбирались работы, содержащие информацию о факторах риска, методах и критериях диагностики, сведения о методах лечения, либо проспективного наблюдения с эктопической стеночной беремен-ностью. Работы распределялись по категориям: клинический случай/серия случаев, обзор, клиническое иссле-дование, рандомизированное контролируемое исследование, систематический обзор/мета-анализ; выделялись абстракты и полноразмерные статьи.Результаты. Обнаружено 1425 источников, соответствующих ключевым словам. Из включенных согласно кри-териям 1006 работ 236 представлены статьями и 770 -абстрактами; в том числе 863 клинических случаев, 129 обзоров, 5 клинических исследований, 4 рандомизированных контролируемых исследования, 5 систематических обзоров. Эктопическая стеночная беременность включает различные формы со специфическими факторами ри-ска, в ряде случаев возможно донашивание беременности до срока жизнеспособности плода. Основным методом диагностики является ультразвуковое исследование. Сроки диагностики и методы лечения не стандартизированы.Заключение. Ввиду редкости патологии и высокого риска серьезных осложнений, включая утрату фертиль-ности и смерть, необходим национальный аудит стеночных эктопических беременностей с формированием базы данных для анализа исходов и расчета индивидуального прогноза. Для повышения информированности врачей о редких формах эктопической беременности и совершенствования стандартов оказания медицинской помощи необходима разработка клинических рекомендаций.Ключевые слова: cтеночная эктопическая беременность, интрамуральная эктопическая беременность, ин-терстициальная эктопическая беременность, эктопическая беременность в трубном углу, шеечно-перешеечная беременность, беременность в рубце после кесарева сечения ABSTRACT Aim. First trimester bleeding occurs in up to 30% of all diagnosed pregnancies. Important causes of first trimester bleeding include spontaneous abortion, missed or threatened abortion, ectopic pregnancy, and gestational trophoblastic disease. One of the greatest dilemmas for clinicians is to accurately diagnose the cause of pain or bleeding, and specifically to determine if an ectopic pregnancy exists due to its grave consequences. Ectopic pregnancy occurs in almost two percent of all reported pregnancies in the United States and is the leading cause of pregnancy-related death in first trimester. When an early pregnant patient is identified who has bleeding or pain, it is crucial step to determine where the pregnancy is located. Ultrasound as a first line diagnostic tool offers an excellent opportunity for pregnancy localization. The use of the beta subunit of human chorionic gonadotropin (beta-hCG) quantification is a valuable adjunct to help determine the course and possible outcome of an early pregnancy. The goal should be to preserve the health and future reproductive capabilities of our patients.
