Diagnosis in Childhood Abdominal Burkitt’s Lymphoma

Abstract: Although the most frequently used technique is laparotomy and open biopsy in our series, other methods provided quicker initiation of chemotherapy and less surgical morbidity. Especially in patients with high stages, cytological evaluation and tru-cut needle biopsy with radiological guidance is a better alternative of laparotomy.

“…First, tissue must be obtained. In general, laparoscopy is used to obtain tissue for histological analysis, but the use of tru‐cut needle biopsy under radiological guidance is gaining popularity due to its ability to provide quicker initiation of chemotherapy and to decrease surgical morbidity . Nevertheless, in patients such as those described here, in whom diagnostic uncertainty is present, laparoscopy and frozen section have the advantage of allowing surgical intervention to take place immediately if necessary.…”

“…High-grade B-cell non-Hodgkins lymphoma has a fast cell turnover leading to rapid growth of tumours and a propensity to tumour necrosis [2,5,10]. These characteristics increase the risk of an acute abdomen not only as an initial presentation but also during the course of treatment.…”

“…Burkitt lymphoma has been diagnosed following laparotomies for appendicitis, bowel perforation, obstruction and haemorrhage [10][11][12]. In one series of 57 B-cell NHL patients with abdominal disease, 67% required initial surgery, nearly half of these due to a surgical emergency [2], the others required surgery for diagnostic purposes or planned resection of small localised tumours.…”

“…Bu durumda olası en az invazif yöntemlerle (laparoskopi, torakoskopi gibi) biyopsi alınmaya çalışılmalıdır. Uygun olgularda gö-rüntüleme kılavuzluğunda kalın iğne biyopsisi ile tanı koymak olasıdır (25) . Ancak İİAB, doku tiplendirmesi yapılabilmesi için yeterli örnek sağlamayacağından NHL tanısında kullanılması önerilmemektedir (8) .…”
