Diagnosis and treatment of sarcoma of the uterus. A review

Abstract: Background. The histopathological classifi cation and staging system for uterine sarcoma (US) were revised in 2003 and 2009, respectively. However, there is currently no consensus on the signifi cance of various prognostic factors. Therefore the available clinicopathological data on US are summarized in this review. Methods. Articles on uterine sarcoma published in English from 1970 to 2011 were identifi ed systematically by computer-based searches in Medline and the Cochrane Library. Results. “…While adjuvant pelvic radiotherapy may be used to reduce the risk of local recurrences, systemic recurrences and metastatic disease may be prevented with chemotherapy. However, the effectiveness of any form of adjuvant treatment on survival has not been consistently shown (1,12). In a Gynecologic Oncology Group (GOG) study, 156 patients with stage I (disease limited to corpus) or stage II (disease limited to the corpus and cervix) uterine sarcomas were evaluated in a randomized study.…”

“…Five-year disease-specific survival for patients with stage I and II disease is 51% and 25%, respectively. For all stages of patients with LMS of the uterus, 5-year disease specific survival is only 32% (1,6).…”

“…Vascular invasion is the only statistically significant prognostic factor, with 5 year survival rates of 83% and 17% in the absence and presence of vascular invasion, respectively. 11,12 Adenosarcomas consist of benign epithelial components and homologous or heterologous mesenchymal sarcomatous components; they are more common in postmenopausal women. However, these tumors can also occur in adolescents and young adults, and they comprise 5.5 to 9.0% of all uterine sarcomas.…”
