Diagnosis and Treatment of Cervical Intraepithelial Neoplasia in a Single Visit

Abstract: We evaluated the diagnostic and therapeutic efficacy of large loop excision of the transformation zone (LLETZ) performed in the first visit (see and treat policy) as compared to LLETZ treatment performed as an interval procedure. Data of 248 patients were analyzed of which 206 patients had LLETZ. Two thirds of the procedures were performed at the first visit. Of all the women who had the 'see and treat policy' 94.9% were diagnosed to have cervical intraepithelial neoplasia (CIN) on histology compared to 90.8% … Show more

“…The number of women with malignancy was less than 1%, which is similar to other colposcopy clinics. 9 In patients where a histology report could not be found, repeat colposcopy was performed and treatment offered, depending on the colposcopic findings. These findings were not analysed further.…”

“…However, one of its disadvantages is potential unnecessary treatments as claimed by other authors. 1,2,3,5,11 The traditional approach with a targeted biopsy for diagnostic confirmation requires more medical visits before the actual treatment of the lesions. 11 This causes increased anxiety to patients, which can in turn lead to dropout before treatment.…”

“…Some have purported that the availability of a tissue specimen is an advantage of LEEP compared to ablative therapy because adenocarcinoma or invasive squamous carcinoma has been identified in a small percentage of LEEP specimens performed in the setting of cervical intraepithelial neoplasia. 2,6,[12][13][14]16,17 In a large study of 1000 women undergoing a single-visit approach, 9 (0.9%) of the LEEP specimens were found to have microinvasive or invasive carcinoma. 14 In another study of 206 patients undergoing single-visit large loop excision of the transformation zone (LLETZ), microinvasive carcinoma was detected in 1.4% of the women in the study.…”

“…14 In another study of 206 patients undergoing single-visit large loop excision of the transformation zone (LLETZ), microinvasive carcinoma was detected in 1.4% of the women in the study. 16 In a ''see and treat''study performed in Peru, of 149 patients randomized to LEEP for the treatment of suspected dysplasia, there were 2 cases (1.3%) of microinvasive carcinoma. 2 In a study in which 915 patients underwent a ''see and treat'' LEEP, there were 15 cases of adenocarcinoma-in-situ and 6 cases of microinvasive squamous carcinoma.…”
