Diagnosis and Management of Lateral Semicircular Canal Benign Paroxysmal Positional Vertigo

Abstract: Adding supine positional testing to routine vestibular diagnostic testing will increase the identification of LSC-BPPV. Apogeotropic LSC-BPPV is more challenging to treat.

“…For the treatment of HSC-BPPV, the barbecue maneuver (and its several variations) is the one that is most commonly used [Fife, 1998;White et al, 2005;Escher et al, 2007], often in association with FPP [Nuti et al, 1998;Casani et al, 2002;Koo et al, 2006] achieving success in 50-100% of the cases. Some years ago, Gufoni et al [1998] described a method of treatment that involves shifting the patient rapidly from a sitting position to the healthy side (in the geotropic forms) or to the affected side (in the apogeotropic forms).…”

“…(2) the apogeotropic type, less common, in which the material is found in the ampullar branch of the canal very near the cupula; by means of specific maneuvers this form (generally more resistant to treatment) can be converted into the geotropic type [White et al, 2005]. If this is not possible, the apogeotropic type may be considered a true cupulolithiasis since the debris probably adheres strictly to the HSC cupula [Baloh et al, 1995;Casani et al, 1997].…”

“…However, this is not particularly surprising because BPPV secondary to cupulolithiasis is often less responsive to conservative treatment measures than canalithiasis. 7 This is presumably because otoconial debris must be freed from the cupula before repositioning maneuvers can be effective. We suspect that this patient has otoconial debris firmly adherent to the cupula, which were not mobilized despite extensive efforts in vestibular rehabilitation therapy.…”
