Diagnosis and management of gastrointestinal complications in adult cancer patients: 2017 updated evidence-based guidelines of the Infectious Diseases Working Party (AGIHO) of the German Society of Hematology and Medical Oncology (DGHO)

Abstract: Cancer patients frequently suffer from gastrointestinal complications. In this manuscript, we update our 2013 guideline on the diagnosis and management of gastrointestinal complications in adult cancer patients by the Infectious Diseases Working Party (AGIHO) of the German Society of Hematology and Medical Oncology (DGHO). An expert group was put together by the AGIHO to update the existing guideline. For each sub-topic, a literature search was performed in PubMed, Medline, and Cochrane databases, and strength… Show more

“…In addition, Gram-negative bacteria are more commonly found as microorganisms causing blood stream infections (BSI) in neutropenic patients, compared with non-neutropenic patients [ 28 , 80 ]. This is likely due to bacterial translocation of gut organisms frequently causing BSI rather than CRI in neutropenic patients [ 81 ]. Increasing rates of antibiotic resistance including multi-drug resistance (MDR) have been reported worldwide in the last decade also in studies focusing on cancer patients [ 82 84 ].…”

“…When any of these protective mechanisms are disrupted, infection can ensue. The gastrointestinal tract is a common site of infection in cancer patients, with causes including Clostridium difficile- associated colitis, cytomegalovirus (CMV) colitis, neutropenic enterocolitis, appendicitis, and intestinal perforation with secondary peritonitis and possibly abscess formation [ 34 ]. Clinical manifestations include abdominal pain, fever, diarrhea, and less commonly intestinal hemorrhage.…”
