The Orthopaedic Section of the American Physical Therapy Association presents this fifth set of clinical practice guidelines on knee pain and mobility impairments, linked to the International Classification of Functioning, Disability, and Health (ICF). The purpose of these practice guidelines is to describe evidence-based orthopaedic physical therapy clinical practice and provide recommendations for (1) examination and diagnostic classification based on body functions and body structures, activity limitations, and participation restrictions, (2) interventions provided by physical therapists, (3) and assessment of outcome for common musculoskeletal disorders. InleidingDe NHG-Standaard Traumatische knieproblemen geeft richtlijnen voor de diagnostiek en behandeling van knieproblemen die het gevolg zijn van een trauma. Deze standaard vormt samen met twee andere standaarden een drieluik over knieproblemen in de huisartsenpraktijk (zie tabel 1). Knieproblemen die niet het gevolg zijn van een trauma worden besproken in de NHG-Standaard Niettraumatische knieproblemen bij kinderen en adolescenten en de NHG-Standaard Niet-traumatische knieproblemen bij volwassenen. 1 In deze standaard worden de volgende aandoeningen besproken: contusie en distorsie, kruis-en collateralebandletsel, meniscusletsel en patellaluxatie. Tevens komt een kniefractuur aan de orde en wordt besproken in welke gevallen een röntgenfoto van de knie moet worden overwogen. Kraakbeenletsels, botkneuzingen, posterolateraleknieletsels en patellapeesrupturen blijven buiten beschouwing omdat deze vrijwel uitsluitend in de tweede lijn worden gediagnosticeerd en een beperkte relevantie voor het beleid van de huisarts hebben of zeldzaam zijn. 2 Tijdens het eerste consult na het trauma wordt beoordeeld of er reden voor het aanvragen van een röntgenfoto of voor directe verwijzing is. In de acute fase na een trauma zijn de indicaties voor verwijzing voor nadere diagnostiek beperkt door het ontbreken van therapeutische consequenties. De huisarts kan het merendeel van de patiënten met traumatische knieklachten zelf begeleiden. Het beleid bestaat uit voorlichting, uitleg over het natuurlijk beloop en instructies ten aanzien van het hervatten van activiteiten. Bij aanhoudende of terugkerende klachten en beperkingen overweegt de huisarts verwijzing naar een orthopedisch chirurg. AchtergrondenBegrippen -Contusie: kneuzing van de weke delen van de knie door stomp letsel. -Distorsie: overmatige rekking van het gewrichtskapsel en eventuele gewrichtsbanden. -Collateralekniebandletsel: partiële of totale scheuring van de mediale of laterale collaterale knieband. -Kruisbandletsel: partiële of totale scheuring van de voorste of achterste kruisband. -Meniscusletsel: scheur of totale breuk in de mediale of laterale meniscus, afhankelijk van het type scheur wordt gesproken van een verticale lengtescheur (Bucket handle of flapscheur), (verticale) radiaire scheur of horizontale (vissenbek) meniscusscheur. Dit laatste type ligt in het vlak van de meniscus en kan een uiting van degeneratie zijn. -Patellaluxatie: luxatie van de patella, meestal naar de laterale zijde. -Extra-articulair knieletsel: contusie, distorsie al dan niet met collateralebandletsel. -Intra-articulair knieletsel: meniscus-of kruisbandletsel eventueel in combinatie met collateralebandletsel. -Slotstand: blokkering van de beweeglijkheid van het kniegewricht met een actieve en passieve extensie-en soms flexiebeperking van de knie. BackgroundThe stump impingement reflex is a subtle bounce to the knee thought to be caused by hamstring contraction when the knee is brought into extension and the torn ACL stump impinges between the distal femur and the tibial plateau. We have studied the diagnostic value of this sign.FindingsFrom Feb 2008-Feb 2009, we audited 30 patients who underwent urgent arthroscopy for acutely locked knees. The presence of the stump impingement reflex prior to surgery was compared with the intra-operative findings. The diagnostic values of the stump impingement sign were found to be: Sensitivity 58%, Specificity 81%, Positive predictive value 70%, Negative predictive value 72% and Accuracy 71%.ConclusionsWe believe that the stump impingement reflex is a specific sign for ACL stump impingement as a cause of knee locking. We recommend close inspection for this sign when examining locked knees.
