Diagnosing Osteomyelitis in Patients With Diabetic Neuropathic Osteoarthropathy

Abstract: A 3-month randomized placebocontrolled trial I n men, an association between lower plasma total testosterone (PTT) and insulin resistance has been found in cross-sectional studies (1,2) and in one nested case-control study (3) without any possible conclusion in terms of causality or direction of the relationship. Indeed, to obtain such information, randomized controlled trials are needed. Until now, only one clinical trial has suggested that testosterone therapy improves insulin sensitivity in obese men (4). C… Show more

“…Clinical and imaging tests show that whole-tissue properties of bone, including hardness and mineralization, are directly affected by diabetic osteomyelitis (1,2). We hypothesized that compositional changes to bone mineral and collagen matrix accompany clinically observable alterations in bone hardness and mineralization.…”

“…As reported by Rajbhandari et al [41••], a sausage toe deformity associated with local neuropathic foot ulcers correctly indicated osteomyelitis in 13 of 14 patients studied, whereas only six revealed abnormal roentgenograms and only seven were associated with abnormal bone scans. Magnetic resonance scans, however, remain the proven gold standard for osteomyelitis evaluation, as reconfirmed in a recent report [42].…”
