Diagnosing occult tumour cells and their predictive value in sentinel nodes of histologically negative patients with colorectal cancer

Abstract: Additional immunohistochemical analysis of histologically negative lymph nodes demonstrates occult tumour cells in 33% of the patients resulting in an upstaging rate of 12%. Occult tumour cells are predominantly found in the SN, therefore SN mapping has the potential to refine the staging system for patients with colorectal cancer.

“…We have demonstrated in a previous study that OTC are preferentially found in peritumoral SNs [5]. Since lymphatic drainage to peritumoral lymph nodes is not disturbed by early ligation, it is unlikely that solely a medial to lateral approach would result in decreased numbers of ITC.…”

“…Based on the sentinel lymph node (SN) concept in which the lymphatic drainage from the primary tumor follows a specific order, we showed that OTC are preferentially found in the SN of patients with colorectal cancer [5]. …”

“…In group A, serial sectioning of all SNs was performed at 500‐μm intervals and three serial sections per level were stained with Ber‐EP4 (Dako, Eindhoven, The Netherlands), CK20 (Euro Diagnostica, Arnhem, The Netherlands) and Cam5.2 (Becton Dickinson, Alphen aan den Rijn, The Netherlands) to reveal occult tumour cells (OTCs). Non‐SNs were not analysed immunohistochemically because we previously demonstrated that OTCs are predominantly found in the SN [14].…”

“…Most studies therefore have reported the use of 2 to 3 H&E-stained serial sections on SLN. 37 In another, 8% of SLN metastases were found only after multilevel sectioning and staining with anticytokeratin and anti-CEA antibodies. In 1 study, 21% of patients with histologically negative SLN had additional disease identified using anti-MNF116 keratin on up to 4 serial sections, although 75% of patients "upstaged" had isolated tumor cells.…”
