Diagnosing causes of headache within the postpartum period

Abstract: Research has shown that many postpartum headaches go undiagnosed and as a result are poorly treated, often returning following discharge from secondary care (Nelson-Piercy 2010 ). The hope is that the introduction of this adjuvant will allow clinicians to identify the cause of postpartum headaches earlier and will allow life threatening diagnoses to be quickly excluded.

“…Emma Stanhope et al [9], in order to establish a standardized approach for the management of patients with postpartum headache, tried to introduce a specific diagnostic tool, the PARTUM mnemonic: Pressure (blood pressure for pre-eclampsia/eclampsia), Anesthetic (post-dural puncture headache), Reversible (vasoconstriction syndrome), Thrombosis (cerebral venous sinus thrombosis, ischemic stroke), Use your brain (there are so many other causes of headache), and Migraine, which was first proposed by Lim et al [10]. The aim of this tool is to enable clinicians to identify the cause of postpartum headaches faster and to allow life threatening diagnoses to be quickly excluded [9].…”

“…Because of the multiple potential etiologies and complications that may evolve, the identification of potentially harmful etiologies is of paramount importance for clinicians who need to quickly exclude or confirm potentially life-threatening diagnoses [9].…”

“…Though the vast majority are caused by primary headache disorders, 4.7% are due to postdural puncture etiologies [1]. As this population is at particular risk for hypertensive and hypercoagulable disorders, other emergent considerations for postpartum headaches in the emergency department (ED) should be considered; these include postpartum pre-eclampsia, cerebral vascular accidents (CVA), venous sinus thrombosis (VST), and reversible cerebral vasoconstrictive syndrome [2][3][4]. Subdural hematomas (SDH) secondary to epidural placement is a rare but serious etiology of postpartum headache, with an estimated incidence of one in 500,000 intrapartum epidural anesthesia placements [5].…”
